This study provides a model for comparing the cost-effectiveness of two mental health programs. The example cited examines the cost-effectiveness of a hospital-based and a community-based partial care program. Using each group as its own control, the authors compare patients' use of inpatient hospitalisation and emergency room services during one year prior to and one year subsequent to first admission in day care. In both day care programs, patients substantially reduced their number of inpatient admissions and days. Neither program, however, showed a significant decrease in patients' use of the emergency room. Reflecting the substantially lower costs of the community-based program, the benefit-cost and cost-effectiveness ratios were significantly better in the community-based program than in the hospital-based program. Based on this evidence the authors conclude that the lower-cost, equally effective community-based program should be carefully considered as an alternative for more partial care patients.  相似文献

A closer look at the rich world of California feminisms demonstrates how Judy Grahn served as a central figure in bay area feminism, working to establish and support lesbian activist organizations, feminist publications, women's cultural events, and more. Two of Grahn's early political writings consider how lesbians sat at the nexus of homophobia and sexism. These writings demonstrate the formative role played by San Francisco lesbians in reframing ideas about “women-loving women” and the intersections of gender and sexuality in creating the oppressions faced by all women.  相似文献

A method is presented for engineering the necessary levels of measurement reliability for evaluating ongoing programs. Two studies of levels of client functioning at a community mental health center (CMHC), in which an outcome measure, the Global Assessment Scale (GAS), did not perform as expected, drew attention to the need for better control of outcome measure reliabilities. Drawing from generalizability theory, a study was conducted of three sources of GAS score variance — clients, raters, and training in the use of the scale. Several estimates of reliability (ERs) were developed, depending on the manner in which the GAS ratings were, or would be, obtained in the CMHC. The differences among these ERs clarified why the GAS had lower reliability when used in our setting. Finally, two hypothetical examples are described to illustrate the utility of applying generalizability theory to achieve higher reliabilities for outcome measures.  相似文献

This article examines the confidence the population (N = 4,003) has in the child welfare system in four countries – England, Finland, Norway and the USA (California). We find that about half or less of the population reports having confidence in the system, which is slightly higher than the confidence in the civil servants in the same countries. The Nordic countries display more confidence in the child welfare system than the Anglo‐American countries. The similarity between the countries is, however, greater than anticipated. As for independent variables that can shed light on differences in confidence levels, we find three variables to be related to a higher confidence level, and these are a left wing political orientation, lower age, and higher education. This study contributes in filling a knowledge gap on studies about trust in the child welfare system, but we emphasize that we have studied an aspect of trust that rests on the population's impressions of a system, and not their substantial knowledge about, or identification with, this system.  相似文献

Although past migration was often viewed with suspicion the implicit assumption was that new migrants would fill the jobs in the growing cities and contribute to the national wealth. There are those who still subscribe to the view that continuing migration will increase national wealth, indeed that new migrants are the work horses of prosperity. There is another view however, which focuses on the local impacts of migration. This view suggests that the migrations of the late twentieth century are in a different context and create substantial burdens on local communities and states. It may be too, that the mass migrations of the late twentieth century will lead to a new pattern of social exclusion, polarization and a new ethnic under-class. The analysis of recent migration in California documents the extent to which that migration is made up of very low skill, low income, and dependent groups and has the potential to create significant local dependency burdens.  相似文献

Morell and Flaherty (1978) present arguments about the current status of evaluative research as a profession. This paper contains a critique, using a process model of professional development, of the perspectives employed by Morell and Flaherty. Issues discussed are the nature and substance of conflicts in evaluative research, including conflicts over methodological perspectives, the role of the evaluator in program planning, and the appropriate uses and users of evaluative information. The paper concludes with a discussion of the needs within the profession of evaluative research if it is to remain viable and responsive to change.  相似文献

Public opinion toward illegal migration to the United States varies considerably across different segments of the population, but little is known about why some individuals hold more liberal attitudes than others. Several hypotheses are scattered throughout the research literature, but they have not been brought together in one place and tested using a common data set. Nor have the limited tests been satisfactory from a methodological standpoint. Instead of using multiple regression, typically analysts have relied on cross-tabulations of the data. This paper tests five hypotheses about attitudes toward illegal immigration and undocumented migrants using public opinion data from southern California. Only weak support is found for a labor market competition hypothesis. There is firmer evidence for hypotheses relating to cultural affinity between respondents and undocumented migrants and to the role of education. Respondents' evaluations of tangible costs and benefits to themselves also influence their assessments of illegal immigration. Finally, the results of this analysis provide additional support for a symbolic politics model of opinion formation when the model is extended to the issue of undocumented migration to the United States.  相似文献
