This study examined harm, hurt, and neglect by caregivers as well as self-neglect and physical and mental health status among 113 lesbian, gay, and bisexual (LGB) older adults aged 60–88 years, who attended community-based social and recreation programs or groups in the United States. Reporting on their experiences with caregivers, 22.1% of the participants experienced at least one type of harm, including physical, emotional, verbal, sexual, financial, and neglectful; additionally, 25.7% of the participants reported they knew LGB older adults who experienced at least one type of harm from his or her caregiver. With regard to self-neglect, 62.8% reported experiencing it; those indicating positive psychological health reported fewer experiences with self-neglect.

ABSTRACT

This supplemental issue of the Journal of Homosexuality presents research that explores a variety of health care issues encountered by lesbian, gay, bisexual, transgender and intersex (LGBTI) population groups in the United States over the 10-year period from 1993 to 2002.

Topics include access to health care, utilization of care, training of medical and mental health providers, and the appropriate preparation of clinical offices and waiting areas. Authors used a variety of community-based public health research methods, including participant and provider surveys and retrospective chart reviews of patients, to develop this body of research, providing a recent-historical perspective on the complex health care and health-related needs of sexual and gender minorities. Particularly for transgender and intersex populations, the state of research describing their health care needs is in its infancy, and much remains to be done to design effective medical and mental health programs and interventions.

ABSTRACT

This article, which introduces the special issue of the Journal of Homosexuality on “Mapping Queer Bioethics,” begins by offering an overview of the analytical scope of the issue. Specifically, the first half of this essay raises critical questions central to the concept of a space-related queer bioethics, such as: How do we appreciate and understand the special needs of queer parties given the constraints of location, space, and geography? The second half of this article describes each feature article in the issue, as well as the subsequent special sections on the ethics of reading literal, health-related maps (“Cartographies”) and scrutinizing the history of this journal as concerns LGBT health (“Mapping the Journal of Homosexuality”).

This study examines attitudes toward gay marriage within the context of concern over the weakening of heterosexual marriage. We use data from a three‐state survey conducted in 1998 – 2000 and designed to explore attitudes toward marriage and divorce reform (N = 976). We find that women, Whites, and younger persons are more approving of gay marriage than men, Blacks, and older persons. Nonparents with cohabitation experience are most approving, whereas parents with no cohabitation experience are most opposed. Heterosexual marriage preservation attitudes are key predictors, net of religiosity and political conservativism. We interpret these findings with theories about vested interest in upholding marriage as an institution and ambivalence resulting from conflicting core values of the sanctity of marriage versus the valorization of individualism.

Many lesbian, gay, and bisexual (LGB) individuals manage the degree to which their sexual orientation is known to others. Visibility management, the process of regulating the exposure of one's orientation, is an important part of the lesbian/gay/bisexual experience in community, family, and virtually all other social settings. The degree to which one allows his or her sexual orientation to be visible can have a profound impact on stress, health, self-esteem, interpersonal relationships, and quality of life. The purpose of the present study was to develop a valid and reliable measure of visibility management. The Lesbian, Gay, Bisexual-Visibility Management Scale was constructed and piloted with a small sample of LGB adults. Results support the potential utility of the LGB scale based on satisfactory evidence of construct validity, item-level discrimination, and subscale reliability.

ABSTRACT

Sexual orientation has been linked to certain health conditions, and yet quantifying sexual orientation in longitudinal studies is challenging. This study examined different methods of accounting for sexual orientation in a cohort study of 300 homeless and unstably housed women followed every 6 months over 3 years. Altogether, 39.7% (= 119) could be considered sexual minority at one or more time points based on identity and/or behavior; 16.3% (= 49) reported shifts in sexual identity. Only 24.0% (= 72) were identified as sexual minority through a single measure of sexual identity, 27.0% (= 81) were identified with a single measure of identity and behavior, 33.0% (= 99) were identified through annual measures of identity and behavior, and 22.0%–22.3% (= 66–67) were identified through latent class analysis including all identity/behavior measures. This study found that sexual fluidity is common in unstably housed women, and many sexual minority women would be missed in longitudinal studies with different methods of accounting for sexual orientation.

This study investigated associations between coming out to parents, experiences of parental support, and self-reported health behaviors and conditions among a population-based sample of LGB individuals using data collected via the 2002 Massachusetts Behavioral Risk Factor Surveillance System (BRFSS; N = 177). We explored the following two hypotheses: 1) Lesbian, gay, and bisexual (LGB) individuals who had never disclosed their sexual orientation to a parent would report higher levels of risk behaviors and poorer health conditions than those who had come out; and 2) among LGB respondents who had come out to their parents, the individuals whose parents had reacted unsupportively would report higher levels of risk behaviors and poorer health conditions than those who had come out to parents who were supportive. Approximately two thirds of gay and bisexual (GB) males and lesbian and bisexual (LB) females reported receiving adequate social and emotional support from the parent to whom they first disclosed their sexual orientation. Among LB females, no disclosure of sexual orientation to a parent was associated with significantly elevated levels of past-month illicit drug use (AOR 12.16, 95% CI 2.87-51.54), fair or poor self-reported health status (AOR 5.71, 95% CI 1.45-22.51), and >15 days of depression in the past month (AOR 5.95, 95% CI 1.78-19.90), controlling for potential confounders. However, nondisclosure to a parent by GB males was not associated with greater odds of any of the health indicators assessed. Among GB males, those with unsupportive parents were significantly more likely to report current binge drinking (AOR 6.94, 95% CI 1.70-28.35) and >15 days depression in the past month (AOR 6.08, 95% CI 1.15-32.15), and among LB females, those with unsupportive parents were significantly more likely to report lifetime illicit drug use (AOR 11.43, 95% CI 2.50-52.30), and >15 days depression in the past month (AOR 5.51, 95% CI 1.36-22.36). We conclude that coming out may be associated with better health for LB women, and that parents who react nonsupportively when their children disclose LGB sexual orientation may contribute to children's increased odds of depression and hazardous substance use.

This paper provides an introduction and overview for the papers in the special issue of this Journal devoted to the latter half of life among gay men, lesbians, and bisexuals. Key issues are highlighted in terms of their impact on gay, lesbian, and bisexual cultural communities: the impact of the large aging population of baby boomers; the significance of age as a social organizer; and the areas of research included in this special issue (intimacy and relationships, HIV and AIDS among elders, and the identities and needs of those growing into middle and later life).

This article describes an integrated model of service provision called a continuum of care (CoC), and illustrates the application of this approach to working with lesbian, gay, bisexual, transgender, and questioning (LGBTQ) youth through a case example. The CoC described in this article includes provision of individual and group counseling, case management, housing/family supports, socialization events, and prevention workshops, along with relevant outcomes. The relevance of a CoC model for LGBTQ youth due to the inclusion of community context and potential to empower service users is explored. Key recommendations for CoC models with LGBTQ youth include a focus on collaboration, provider education, and affirmative practice, and incorporating service user feedback.
