This paper offers a new method for testing one‐sided hypotheses in discrete multivariate data models. One‐sided alternatives mean that there are restrictions on the multidimensional parameter space. The focus is on models dealing with ordered categorical data. In particular, applications are concerned with R×C contingency tables. The method has advantages over other general approaches. All tests are exact in the sense that no large sample theory or large sample distribution theory is required. Testing is unconditional although its execution is done conditionally, section by section, where a section is determined by marginal totals. This eliminates any potential nuisance parameter issues. The power of the tests is more robust than the power of the typical linear tests often recommended. Furthermore, computer programs are available to carry out the tests efficiently regardless of the sample sizes or the order of the contingency tables. Both censored data and uncensored data models are discussed.  相似文献

BackgroundA number of studies have found increased use of complementary and alternative medicine (CAM) during pregnancy and birth. However, little is known about women's motivation in seeking CAM during pregnancy or their experiences of use in relation to their pregnancy and childbirth journey.MethodsA narrative study sought to explore the meaning and significance of CAM use in pregnancy from the perspective of CAM users. Narrative style interviews were conducted with 14 women who had used a range of CAMs during pregnancy and birth. Data analysis focussed on the meaning and significance of CAM use in pregnancy and a number of core themes emerged.FindingsThis paper focusses on the theme which illustrates the meaning behind women's use of CAM in pregnancy and childbirth as one of seeking holistic wellbeing.ConclusionParticipants engaged with CAM as a way of fulfilling their physical, emotional and spiritual needs during pregnancy. Use of CAM signified women's desire to be proactive in health seeking behaviours.  相似文献

Public policy discussions on how to regulate acupuncture and herbal medical practitioners have reached a stalemate in the United Kingdom. After considerable activity in the first half of the current decade a wider review of professional regulation has re-opened the discussion as to the most appropriate way to regulate the practice of all health-care practitioners. In the meantime, the public continues to consult complementary and alternative medicine (CAM) practitioners in large numbers and self-medicate with herbal products and other natural remedies, posing challenges for policy-makers as to how to ensure public safety. In the NHS, providers and purchasers struggle to reconcile demands for access to CAM services with their clinical governance requirements. Hong Kong implemented new arrangements for the statutory regulation of traditional Chinese medical practitioners in the 1990s and has experienced the challenges of regulating a large established private market as well as integrating Chinese medicine further into the public health system. This experience is analysed in order to see whether the approach adopted there could address the public policy challenges faced in the UK. The article finds that, despite key cultural and historical differences related to the provision and use of CAM services, the similarities between the health-care systems and the reasons for moves to professional regulation in Hong Kong and the UK provide useful insights into what is happening in the UK in relation to service provision and the relationship with the NHS and the medical profession. These are discussed in the light of current health policy developments in the UK.  相似文献

SUMMARY

The psychological experiences of lesbian mothers, both coupled and single, are compared and contrasted with heterosexual and gay parents who use assisted reproductive technology, focusing on issues of parental desire, fertility, babies conceived from science rather than sex, presence of an outside party in conception, genetic asymmetry, social anxieties, legal protections, disclosure, and gender. The psychological meaning of the donor or surrogate as an “extra” and “missing” piece of the family, along with the interactive effects of homophobia and “reproductive technophobia” are considered. Lesbian families are recognized to be constructing a new narrative of a bio-social family as they define and live their experience.  相似文献

In the wake of the recent economic challenges, it is clear that social workers must be educated about the financial opportunities for low-income households. Encouraging low-income households to have a bank account at an insured depository institution, or become banked, is increasingly emphasized in policy and practice. Becoming banked offers the opportunity to avoid high-cost alternative financial services. Social workers can facilitate relationships between low-income households and the mainstream financial services industry to help families advance their long-term financial interest. Understanding the structure of all types of banks and credit unions is imperative. Implications for social work practice are explored.  相似文献

Abstract

By reading The Chickencoop Chinaman and M. Butterfly together, this paper argues that they construct the Chinese male subject in pronouncedly different if not opposed ways – one by overplaying ‘masculinity’ to comic proportions out of an anxiety of desexualization and the other by overplaying ‘effeminacy’ to the point of female impersonation that questions the ‘intrinsic truth’ of gender itself – thus articulating the divided reaction of a raced and gendered community to a historic condition it had long been mired in. I would also argue that both plays are fraught with inner contradictions in their conceptualiza- tion of the subject. In Chickencoop, the male subject becomes a prisoner of normative Western manhood, which has simultaneously been challenged by black as well as Chinese conceptions of masculinity, and therefore remains inconsistent in its perception of its own maleness. And in M. Butterfly male subjectivity fails ultimately to transcend the framework of normative hetero- sexuality that it had played with all along. Yet, when it comes to the castrating impact of hegemonic Western masculinity on the Chinese/Oriental man, it is M. Butterfly which self-reflexively suggests greater possibilities for him to move beyond the restrictive boundaries of gender/sexuality and experiment with alternative masculinities. The paper examines in the process the relationship of homosociality and effeminacy to homosexuality in Chinese culture in China as well as in the Westthe Westthe West.  相似文献

SUMMARY

When gay and lesbian couples decide to become parents, they are unique as a group in always requiring the involvement of a facilitating other: a donor, surrogate, or (in the case of adopted or foster children) birth parents. This clinical paper explores common psychological and social challenges gay and lesbian couples face when using alternative reproductive technologies to attain parenthood. Between the wish and the actuality of being at home with their baby, gay and lesbian parents travel a long and winding road of choices and chances taken. The parenting partners often consist of one biological and one non-biological parent. Issues of psychological/emotional parenthood as opposed to merely biological parenthood (including assumptions of potential inequality or differential legitimacy) must be reconciled in the minds, couple relationships, family of origin relationships, and friendship support systems of the partners before and after the child's birth. The family must also navigate others' questions and assumptions as they venture ever further beyond their intimate circle and as their growing child forms relationships with peers. Specific guidelines are offered for helping couples surmount these psychosocial challenges.  相似文献

Social science has recently examined the dramatic increase of witchcraft and magic in everyday contemporary African. A study, which took place in the 1970's, on the representation of madness in postcolonial Congo, contributes to the elucidation of such an outgrowth. In line with the first version of La Psychoanalyse, it aimed at identifying variations in the images, beliefs, and attitudes associated with groups whose social positioning differed in relation to modernity. Sixty old men were interviewed. The respondents provided a representation in the making that neither reflected Western knowledge nor faithfully echoed local patterns. The Western elements were anchored in a strongly objectified local belief system. For “traditional” informants the meaning attributed to madness testifies to the transformation of a hegemonic representation into a polemical one since it addressed the question of their identity shaken by modernity. An emancipated representation emerged within the most educated group. A secondary analysis of the data contributes to current theoretical debates within social representations theory in focusing on tolerance/intolerance to alternative representations through semantic barriers. It brings more evidence to the fragmentation of the hegemonic system of belief and confirms how social—identity content and relations mediate knowledge construction.  相似文献
