The incidence of pregnancy during adolescence has escalated dramatically in recent years, with an estimated half a million infants born to teenagers each year in the United States. A disproportionately high amount of child abuse and/or neglect is encountered among adolescent parents, underscoring the need for an effective intervention strategy. Short-term psychotherapy may be beneficial in these cases for forging a therapeutic alliance expeditiously and rigorously challenging the patient's resistance. The case described involves alleged abuse by a 16-year-old mother. An intervention was designed to clarify the allegations surrounding the abusive incident, help the mother confront episodes of loss in her own life and overcome feelings of hopelessness engendered by separation from her infant. 相似文献

Active self-reflection is a sophisticated and subtle, multi-layered process that requires learning the capacity to constantly self-monitor. These fundamental interactive skills, the core of a competent social worker, are important to work in any context. Using two distinct metaphors, one relating to horses, the other to baseball, the article will explore the importance of developing competency skills and utilize a ‘third eye.’ In current social work education curricula the use of self may not be prioritized, practiced, or well understood. Evidence from human and veterinary medicine as well as social work, are discussed. An example of an undergraduate BSW course using simulated clients and standardized measures is discussed. The paper includes a brief review of the preliminary findings for a small pilot study and the relevance to future research is considered. 相似文献

There is a significant body of research into the benefits of animal-assisted therapy (AAT) but less into the fields known as equine-assisted learning and therapy (EAL/EAT) where horses are incorporated in therapeutic and learning interventions. This paper explores the experiences of seven ‘at-risk’ young people who participated in a therapeutic horsemanship (TH) programme. The study followed a practice-near approach seeking to capture the young people's experiences within a participative ethnography. Themes related to the risk and resilience literature such as self-confidence, self-esteem, self-efficacy and a sense of mastery, empathy and the opening of positive opportunities are explored in this paper. 相似文献

Open Dialogue is an approach to working with people and their families experiencing psychosocial distress. Interest in Open Dialogue in Australia has been growing recently, raising questions about its adaption and implementation to local contexts. This article is an attempt to answer some of the frequently asked questions we have encountered in training and discussions about Open Dialogue. We attempt to provide responses to questions of how Open Dialogue is different to what is done already, how it fits with current approaches, how you know if you are doing it, whether it is passive or just about doing reflections, issues about including the social network, and concerns about the evidence base. This article aims to present a variety of viewpoints in relation to these questions and to hopefully further discussions on how Open Dialogue can be implemented and adapted to Australian health care and social care contexts. 相似文献

ABSTRACTEngagement of individuals with serious mental illnesses in community mental health services is a significant challenge. The Program of Assertive Community Treatment (PACT) is an individual-centered and self-contained mental health program that provides psychiatric treatment, rehabilitation, and support services to persons with serious mental illness who have a history of or likelihood of disengagement with services. Understanding what helps and hinders consumers’ involvement in PACT services may provide information on how to tailor engagement strategies to individuals based on their treatment needs and preferences. The current study builds on existing studies by exploring factors that help and hinder engagement in PACT services from the perspectives of individuals receiving treatment. We conducted open-ended, semi-structured interviews with 17 individuals receiving PACT services. Nine themes were identified through thematic analysis: Desirable qualities of PACT, Focusing on positive outcomes, PACT as a safety net, Recovery orientation, Practical barriers, Conflictual relationships, and Medication side-effects. We conclude that the development of a trusting, therapeutic relationship that is collaborative and person-centered and that is facilitated by both practical and emotional support is critical to engaging individuals in treatment and maximizing positive outcomes. 相似文献

Rhetorical questions emerging spontaneously in consultations can be used as a tool for developing self‐reflexivity and exploring the therapeutic alliance in the process of therapy with couples and families. This paper presents a technique based on the enunciation of rhetorical questions, which can point to an impasse in the process of therapy and contain a key to its resolution. The technique focuses on the distinctive feature of rhetorical questions as convening a paradoxical injunction: a question not intended as a question, while uttered as such. The technique in four steps is illustrated by examples from therapy, supervision, consultation, and self‐supervision. Its focus is the interlink between the therapeutic relationship and the process of therapy and it is informed by a second‐order cybernetics approach, dialogical practice, and the systemic literature on emotion. The technique can help develop curiosity in working with families, lead to unforeseen developments, and touch on prejudices not made explicit in previous therapeutic encounters. Whether we are acting as supervisors, therapists, or clients, the creation of a ‘secure enough’ context for rhetorical questions becomes critical to allow freeform exploration. 相似文献

Deception, in its many forms, is present in families and relationships across the globe. The impact of the presence and discovery of family secrets or deceptive acts on the individual, the family unit, and the therapeutic process, particularly in the field of marriage and family therapy, has not been thoroughly studied or discussed. Further, deceptive acts during the therapeutic process may have implications for effective treatment planning. This article defines the various forms of deception and its systemic effects on relationships and on the therapeutic process. The present study is part of a series of qualitative studies focusing on dialogic practice in southern Norway. In this article, we present a qualitative study of a network meeting focusing on the interplay between the participants' inner and outer dialogues. The network meeting is between an adolescent boy, his mother and two network therapists, the same adolescent case discussed previously in this journal by Bøe et al. ( 2013 ). The aim of this study is to explore how the interplay between inner and outer dialogues contributes to significant and meaningful moments for the interlocutors. A multiperspective methodology is used that combines video recordings of a network meeting and participant interviews with text analysis. Our research found the interplay has an important role in understanding the emergence of significant and meaningful moments in therapy. A one‐sided focus on participants' utterances or inner dialogues was insufficient to explain their significance and meaning to the interlocutors. A dialogical approach provides a theoretical frame and concepts that are useful in investigations of therapeutic conversations. 相似文献
