Children and Society Policy Review: Health Policy Affecting Children and Young People

The Health and Social Care Act comes into force in April 2013. It changes the organisation of the health service and accelerates the integration of health and social care. New relationships between primary and secondary healthcare will develop and the culture of clinical and cost effectiveness will expand into social care; work on children in public care is in the vanguard of this change. However, this is not an organisational change designed for children and there are considerable anxieties about how it will impact on the delivery of health care. The issues for children in public care need to stay high on the national agenda and in local fora.     

The Role of Design in Inhibiting or Promoting Use of Common Open Space

Abstract

Redwood Gardens is a housing project for the elderly on the Clark Kerr campus affiliated with the University of California at Berkeley. A post occupancy evaluation study done in 1994 indicated that outdoor spaces were not well used, and a follow-up set of interviews and observations over a decade later in 2005 shows that the spaces are still underutilized, despite being greatly appreciated as an amenity. This paper explores reasons for the under usage, describes the design of the spaces, and makes proposals for improvements in seating and other environmental props that might facilitate both solo and group use of outdoor spaces. Design and policy recommendations are directed both to this immediate site and other such housing sites elsewhere.     

Moving to and Living in a Retirement Home

Abstract

Bad health combined witii deteriorating physical functions force many elderly people to move to a retirement home. The primary focus of this study was to investigate the resident's experience of safety and security. From a sample of 57 residents, 12 persons volunteered to participate in the interview. Data were analyzed with qualitative content analysis. The standard and design of the participant's previous home made mobility difficult and this affected their sense of security. This motivated them to move into an environment where they felt more secure. The environment at the retirement home was predominantly described as secure and safe. The finding shows that the participants felt that safety and security were more important than independence.     

The Role of Housing in Community Health Promotion Among the Aged

Abstract

A total of 89 aged residents living independently in single and double occupancy housing in an historic, traditionally planned neighborhood in New Orleans were surveyed with respect to their engagement in the outdoor realm in their immediate neighborhood. An index was developed, comprised of variables spanning the scale of the private dwelling to neighborhood scale. Certain dwelling attributes, health status factors, lifestyle factors, and neighborhood factors were found to be associated with a disinclination to walk outdoors in the community. Among the findings, improperly designed porches and insufficient semi-private exterior space adjacent to the dwelling function as strong deterrents to health-promoting walking activity outdoors, closely followed by fear associated with being victimized by crime in one's immediate neighborhood. Such conditions were found to pose a barrier to full engagement with the community in what on the surface would otherwise appear to be an imminently pedestrian-scaled residential setting. Study limitations, and directions for future research within the environmental design disciplines are discussed for both historic neighborhood settings and in new development.     

Sibling Relationships in Middle and Older Adulthood

Abstract

Research on various aspects of the sibling relationship in adulthood, with a focus on middle and older adulthood, will be reviewed. Attachment theory, which has often been utilized to explain relationships among siblings, will be discussed. Equity theory, as it relates to siblings' roles as caregivers, will also be examined. Closeness among siblings, social support, elderly siblings, and death of a sibling are among the specific topics that will be addressed. A number of factors contributing to the quality of the sibling relationship will also be considered. These include the gender and marital status of the siblings. Finally, future research ideas concerning adult siblings will be suggested.

A burgeoning literature on narrative identity has emerged during the last decades simultaneously with a “performative turn” in the methodology of qualitative social research; both changes indicate a move away from the paradigm of “representation” that emphasizes linear and singular interpretation of “facts” and toward an acknowledgment of the complexity of the social world preferring a dialogic space, open for multiple interpretations and voices. This article aims to explicate how a nonrepresentational narrative stage performance, “We Are All the Same,” by a group of people who had suffered from mental illness opened up space that made transformation possible.     

A Qualitative Exploratory Study on the Service Delivery System for the New Long-Term Care Insurance System in Korea

ABSTRACT

To cope with the rapid growth in its aging population, the Korean government introduced a new social long-term care insurance (LTCI) system for elderly people beginning in July 2008. This study aims to understand how National Health Insurance Corporation (NHIC) staff and home-visiting service providers experienced and evaluated the new service delivery system. A total of 26 semistructured in-depth interviews were conducted with NHIC staff primarily responsible for the functional assessment of the elderly and the home-visiting service providers responsible for providing direct services to elderly clients. In summary, interviewees had negative experiences with the new service delivery system such as difficulties in carrying out their roles. The findings suggest that Korea's new LTCI service delivery system faces challenges and that a more active role for the Korean government, especially regarding the introduction of a proper care management system, is needed to address the issues. Future research with larger sample sizes is needed to examine and understand the issues in detail.     

Living with Elder Wisdom

Abstract

This study is based on interview research with over a hundred creative elders. Their spirituality is explored through their life experiences as re-interpreted in later years. Spirituality is often expressed in “non-religious” language. They speak of inner empowerments: cultivating self-esteem, harvesting memories, transformative turning points, life-long learning, themes of humor and gratitude, and encountering mortality. They also explore outward empowerments: developing new purposes, welcoming possibilities, fostering more freedom, cultivating family and friends, forming intentional communities, and taking on larger social causes re peace, justice and ecology. This elder spirituality manifests important changes in their earlier views of religion.
