新医改背景下医院人力资源管理成效分析

目的:分析某基层医院人力资源管理效果及影响因素,针对其存在问题提出政策建议。方法:采用典型调查方法,对某医院病房医护人员进行问卷调查。结果:该院医护人员薪酬满意度为中上水平,对医院文化建设认同度为中等水平,培训效果欠佳。主要影响因素是医院文化、部门凝聚力、考核制度和对人力资源管理的重视程度。结论:基层医院应制定薪酬分配和考核体系,加强医院文化建设,合理设计培训内容,强化临床技能培训,完善人才引进政策,建立内部培养机制。     

Partial Hospitalization Treatment for Clinical Depression

Abstract

This article describes an outcome study evaluating the effectiveness of a partial hospitalization program (PHP) on the treatment of clinically depressed clients. Within the context of a relatively non-intrusive one-group pretest-posttest research design, nine adult clients completed the Generalized Contentment Scale (a measure of non-psychotic depression) at admission to, and discharge from, the PHP. The clients experienced statistically significant and clinically meaningful improvements between admission and discharge from the PHP. This MSW student-conducted program evaluation represents the type of easy-to-conduct outcome studies which are increasingly being called for in today's practice environment.     

“Sensory Ordering” in Nurses' Clinical Decision‐Making: Making Visible Senses,Sensing, and “Sensory Work” in the Hospital

The objective of this article is to present a study on the constitutive role of senses in clinical decision‐making. The methodology is based on a series of focus groups with nurses in various hospital departments. Based on a narrative approach, our study examines “sensory work” in clinical decision‐making in order to reveal its specificity in the clinical work of nurses. Nurses shared stories—in focus groups—about the influence of senses in clinical decision‐making. The analysis of clinical narratives helped to identify various situations revealing the “sensory work” that underlines clinical decision‐making. We put the emphasis on the spectrum of sensory activities and the interactions occurring during a clinical decision‐making. One specific contribution of our study is to make visible the “sensory ordering” at work as constituted by interactions between nurses during a clinical assessment.

新医改下大型综合医院新型住院难问题研究

自2009年推行新医改以来,由于江苏部分地区在基层医疗机构率先实行了工资改革,出现了基层医务人员将患者推往大医院治疗的不良现象,从而造成了大医院住院难问题.老问题还未彻底解决,新问题已呈现.文章旨在通过对新医改后出现的大医院新型住院难现象进行研究,及时发现新医改过程中出现的问题,为全国新医改的顺利推进提供政策建议.     

小康型乡镇卫生院间医疗资源共享模式探析

目前乡镇卫生院在医疗资源配置中存在人才匮乏、资源配置不合理、服务水平低下、信息化建设滞后、重医轻防等问题,与小康型乡镇卫生院的建设目标存在较大差距。为此,应着力打造乡镇卫生院卫生服务管理集团,积极整合医疗资源,实现乡(镇)村医疗资源一体化运作模式,从而达到乡镇卫生院机构优势互补和资源共享,满足患者的医疗需求。     

生理-心理-社会的结合还是整合?——精神病医院社会工作服务模式探索

随着我国社会工作职业化和专业化的发展,精神病医院已成为我国社会工作开拓的重要领域.本文通过文献回顾和对一所市级精神病医院的个案研究发现,目前国内精神病医院的康复服务已经逐渐迈向生理-心理-社会的综合模式,但仍面临患者比较被动、服务活动松散、社会支持缺乏等难题.出现这种问题的根本原因,是运用病症的视角规划和设计精神疾病患者的心理和社会功能康复的活动,使患者成为被动的参与者.在这样的处境下开展精神病医院社会工作,作为社会工作者首先需要转变服务介入的视角,以精神疾病患者的能力和发展为核心,通过尊重、合作的方式挖掘和调动患者的能力,围绕患者的发展要求与患者一起规划心理和社会方面的不同活动,增强患者自我决定和选择的能力,加强患者与家人以及其他人之间的社会支持,将生理、心理、社会不同方面的服务整合起来.     

Utility of Hospital Emergency Department Data for Studying Intimate Partner Violence

The authors examine 12 months of emergency department visit data (N= 2,521) from the National Electronic Injury Surveillance System All Injury Program and explore its utility for measuring and studying intimate partner violence. Given the dearth of national data on intimate partner violence–related injury and its potential value for public health surveillance and prevention, the data set appears promising for estimating national rates of emergency department visits. Missing perpetrator‐patient relationship data limit estimation of intimate partner–related hospital visits for injury and pain, which precludes national rate estimation at this time, but the data are still useful for describing documented intimate partner violence cases and may be helpful in designing prevention strategies.

The Baby as Subject: The Hospitalised Infant and the Family Therapist

This article is concerned with the experience of infants who endure long hospitalisations. It explores the concept of the baby as subject; that is, infants as active, psychic participants in their earliest relationships. It explores therapeutic possibilities with hospitalised infants from an infant mental health perspective. It advocates for family therapists to equip themselves for infant—parent therapeutic work.
