Integrating phase 2 into phase 3 based on an intermediate endpoint while accounting for a cure proportion—With an application to the design of a clinical trial in acute myeloid leukemia

For a trial with primary endpoint overall survival for a molecule with curative potential, statistical methods that rely on the proportional hazards assumption may underestimate the power and the time to final analysis. We show how a cure proportion model can be used to get the necessary number of events and appropriate timing via simulation. If phase 1 results for the new drug are exceptional and/or the medical need in the target population is high, a phase 3 trial might be initiated after phase 1. Building in a futility interim analysis into such a pivotal trial may mitigate the uncertainty of moving directly to phase 3. However, if cure is possible, overall survival might not be mature enough at the interim to support a futility decision. We propose to base this decision on an intermediate endpoint that is sufficiently associated with survival. Planning for such an interim can be interpreted as making a randomized phase 2 trial a part of the pivotal trial: If stopped at the interim, the trial data would be analyzed, and a decision on a subsequent phase 3 trial would be made. If the trial continues at the interim, then the phase 3 trial is already underway. To select a futility boundary, a mechanistic simulation model that connects the intermediate endpoint and survival is proposed. We illustrate how this approach was used to design a pivotal randomized trial in acute myeloid leukemia and discuss historical data that informed the simulation model and operational challenges when implementing it.     

The Bleaching Syndrome Among People of Color

ABSTRACT

In the aftermath of mass migrations from Europe, most Americans have been of European descent, i.e., “white.” From said migration light skin evolved as the American ideal not irrelevant to people of color. Using a sample of African-American college freshmen, hypotheses were formulated to assess extent of the problem and by inference the Bleaching Syndrome. According to these data, there is a statistically significant relationship between self-identified skin color correlated with light skin providing evidence of the Bleaching Syndrome (the conscious awareness of the cognitive and attitudinal levels of the similarities and differences between the dominant group mainstream and dominated out-group to negate one's self for the purposes of assimilation). Those who study people of color are then challenged to decipher the maze of tradition and create a suitable climate for the study of human behavior in the social environment. In this they will accommodate social justice and an overall ability of diverse groups to assimilate.

Perceived Competencies of Social Workers in Adolescent Practice

Abstract

This investigation yielded data on the perceived competencies of social workers in adolescent practice. On average, survey respondents related that their perceived knowledge/skill levels ranged from moderate to high when working with depressed, sexually abused youths, or those with behavioral issues. However, respondents also perceived their knowledge/skill levels as moderate to poor when adolescent clients presented with: chronic physical conditions including HIV+/AIDS, sexual orientation issues, gang involvement, or were youths of color. Survey respondents credited work experience and conferences as the primary sources of their perceived practice knowledge/skill. Greater knowledge/skill levels in adolescent practice were associated with greater years of practice.

Do you see me? The complex experiences of women of color MSW students

Universities have increased the number of female students of color. However, inconsistency remains in terms of the number of women of color in positions of power and privilege. This discussion attempts to forward the dialogue about the challenges as well as the resilience of female students of color based on the experiences of four female African-American students in a graduate social work program at a predominantly white institution (PWI). Participants shared their daily experience of isolation and exclusion in their graduate programs compared to their experience in Historically Black College or Universities (HBCUs). Participants also discussed the divide that exists in institutions, as well as the sustaining forces and implications for education and research.
