In this work, we study the environmental and operational factors that influence airborne transmission of nosocomial infections. We link a deterministic zonal ventilation model for the airborne distribution of infectious material in a hospital ward, with a Markovian multicompartment SIS model for the infection of individuals within this ward, in order to conduct a parametric study on ventilation rates and their effect on the epidemic dynamics. Our stochastic model includes arrival and discharge of patients, as well as the detection of the outbreak by screening events or due to symptoms being shown by infective patients. For each ventilation setting, we measure the infectious potential of a nosocomial outbreak in the hospital ward by means of a summary statistic: the number of infections occurred within the hospital ward until end or declaration of the outbreak. We analytically compute the distribution of this summary statistic, and carry out local and global sensitivity analysis in order to identify the particular characteristics of each ventilation regime with the largest impact on the epidemic spread. Our results show that ward ventilation can have a significant impact on the infection spread, especially under slow detection scenarios or in overoccupied wards, and that decreasing the infection risk for the whole hospital ward might increase the risk in specific areas of the health‐care facility. Moreover, the location of the initial infective individual and the protocol in place for outbreak declaration both form an interplay with ventilation of the ward.  相似文献

The minimum variance unbiased estimators (MVUEs) of the parameters for various distributions are extensively studied under ranked set sampling (RSS). However, the results in existing literatures are only locally MVUEs, i.e. the MVUE in a class of some unbiased estimators is obtained. In this paper, the global MVUE of the parameter in a truncated parameter family is obtained, that is to say, it is the MVUE in the class of all unbiased estimators. Firstly we find the optimal RSS according to the character of a truncated parameter family, i.e. arrange RSS based on complete and sufficient statistics of independent and identically distributed samples. Then under this RSS, the global MVUE of the parameter in a truncated parameter family is found. Numerical simulations for some usual distributions in this family fully support the result from the above two-step optimizations. A real data set is used for illustration.  相似文献

When using nonparametric methods to analyze factorial designs with repeated measures, the ANOVA-type rank test has gained popularity due to its robustness and appropriate type I error control. This article proposes power and sample size calculation formulas under two scenarios where the nonparametric regression coefficients are known or they are unknown but a pilot study is available. When a pilot study is available, the formulas do not need any assumption on the underlying population distributions. Simulation results confirm the accuracy of the proposed methods. An STZ rat excisional wound study is used to demonstrate the application of the methods.  相似文献

A linear pivotal is introduced as a generalization of an estimate. The defining feature of a linear pivotal is that it generates a likelihood function in the same way as does a statistic. This allows the efficiency and sufficiency of the pivotal to be examined. The efficient reduction of data to a linear pivotal can often be achieved when it cannot be achieved using an estimate. In this paper we introduce a family of test statistics for testing complete symmetry in three-dimensional contingency tables based on phi- divergence families. These test statistics yield the likelihood ratio test and the Pearson test statistics as special cases. Asymptotic distribution for the new test statistics are derived under both the null and the alternative hypotheses. A simulation study is presented to show that some new statistics offer an attractive alternative to the classical Pearson and likelihood ratio test statistics for this problem of complete symmetry.  相似文献

Score statistics utilizing historical control data have been proposed to test for increasing trend in tumour occurrence rates in laboratory carcinogenicity studies. Novel invariance arguments are used to confirm, under slightly weaker conditions, previously established asymptotic distributions (mixtures of normal distributions) of tests unconditional on the tumor response rate in the concurrent control group. Conditioning on the control response rate, an ancillary statistic, leads to a new conditional limit theorem in which the test statistic converges to an unknown random variable. Because of this, a subasymptotic approximation to the conditional limiting distribution is also considered. The adequacy of these large-sample approximations in finite samples is evaluated using computer simulation. Bootstrap methods for use in finite samples are also proposed. The application of the conditional and unconditional tests is illustrated using bioassay data taken from the literature. The results presented in this paper are used to formulate recommendations for the use of tests for trend with historical controls in practice.  相似文献

In applications of generalized order statistics as, for instance, reliability analysis of engineering systems, prior knowledge about the order of the underlying model parameters is often available and may therefore be incorporated in inferential procedures. Taking this information into account, we establish the likelihood ratio test, Rao's score test, and Wald's test for test problems arising from the question of appropriate model selection for ordered data, where simple order restrictions are put on the parameters under the alternative hypothesis. For simple and composite null hypothesis, explicit representations of the corresponding test statistics are obtained along with some properties and their asymptotic distributions. A simulation study is carried out to compare the order restricted tests in terms of their power. In the set-up considered, the adapted tests significantly improve the power of the associated omnibus versions for small sample sizes, especially when testing a composite null hypothesis.  相似文献
