Background

Pregnancy, labour and neonatal health outcomes for Australian Aboriginal women and their infants are frequently worse than those of the general population. Provision of culturally competent services may reduce these differences by improving access to timely and regular antenatal care. In an effort to address these issues, the Aboriginal Maternity Group Practice Program commenced in south metropolitan Perth, Western Australia, in 2011. The program employed Aboriginal Grandmothers, Aboriginal Health Officers and midwives working in a partnership model with pre-existing maternity services in the area.

Aim

To identify elements of the Aboriginal Maternity Group Practice Program that contributed to the provision of a culturally competent service.

Methods

The Organisational Cultural Competence Assessment Tool was used to analyse qualitative data obtained from surveys of 16 program clients and 22 individuals from partner organisations, and interviews with 15 staff.

Findings

The study found that the partnership model positively impacted on the level of culturally appropriate care provided by other health service staff, particularly in hospitals. Two-way learning was a feature. Providing transport, team home visits and employing Aboriginal staff improved access to care. Grandmothers successfully brought young pregnant women into the program through their community networks, and were able to positively influence healthy lifestyle behaviours for clients.

Conclusion

Many elements of the Aboriginal Maternity Group Practice Program contributed to the provision of a culturally competent service. These features could be considered for inclusion in antenatal care models under development in other regions with culturally diverse populations.  相似文献

Uncovering both the structural causes and experiences of suffering is a central sociological endeavor. Sociologists study many different kinds of suffering; after all, strife is experienced both physically and emotionally, because of internal factors such as illness, due to external factors such as trauma, and as a result of economic, political or natural environments. In this paper, I address one form of suffering: mental suffering. In particular, I describe the medicalization of mental suffering in biological psychiatry, which focuses on the genetic factors of illness and equates mental suffering with mental illness. The psychiatric concept of mental illness highlights the continuing, crucial role for sociology in both understanding the experience and identifying the structural roots of suffering. Since the dominant conceptualization of mental suffering is as a medical concept, it is vital for sociology to offer alternative explanations and contribute to a multidimensional analysis. The roots of mental suffering are much more than biological; social comparison, social inequality, and other social stressors are equally important etiological considerations. Therefore, a true understanding of mental suffering requires multiple perspectives, and sociological constructs guard against a total medicalization of mental suffering.  相似文献

Social-emotional intelligence (SEI) has been linked with a number of health behaviors in adolescent populations. However, little is known about the influence of SEI on sexual behavior. This study examined associations between three indicators of SEI (intrapersonal skills, interpersonal skills, stress management skills) and adolescent girls’ sexual risk behaviors. Data come from a cross-sectional sample of sexually active adolescent girls (ages 13 to 17 years) at high risk for pregnancy (N = 253), recruited from health care clinics in a Midwest metropolitan area during 2007 and 2008. Results of multivariable regression models controlling for participants’ age and race/ethnicity indicated that each aspect of SEI was related to distinct sexual risk behaviors. Specifically, girls with greater intrapersonal skills had significantly fewer male sex partners in the past six months (b = ?0.16). Participants with greater interpersonal skills reported earlier communication with their sexual partner about sexual risk (b = 0.14), and those with a better ability to manage stress reported more consistent condom use (b = 0.31). Study findings suggest that SEI may provide a protective buffer against sexual risk behaviors. Building adolescent girls’ social and emotional skills may be an effective strategy for reducing their risk for early pregnancy and sexually transmitted infections.  相似文献

To better understand leadership in public relations and explore its value in successful communication management, this article captured the perceptions of mid- and senior-level public relations executives (N = 222) regarding the important qualities and dimensions of excellent leadership in the practice. According to study participants, strategic decision-making capability, problem-solving ability, and communication knowledge and expertise are the three most important qualities of excellent leadership. In addition, respondents said that on-the-job experiences, individual initiative and desire, and role models are the most valuable sources of leadership skills and development. About half of the respondents indicated that excellent leaders in public relations are different from leaders in other fields in three ways: They must hold a compelling vision for communication, possess comprehensive understanding of media and information systems, and effectively develop and implement strategic communication plans. Professional women and men in the study viewed excellent leadership in more or less similar ways. The study advances understanding of leadership in the field, which has been little explored in the public relations literature.  相似文献

The literature on serving the rural elderly has focused on rural elders primarily as passive recipients of care. This article views rural older women as active manipulators of the social support system within which they meet their perceived needs and the needs of others in their social network. It is based on the findings of a multi-phase qualitative study of 30 older women in central Minnesota. Data were collected through life history interviews, in-depth structured interviews, observation and network analysis profiles. The informants relationships with family, friends, neighbors and formal service providers are discussed. Implications for formal service delivery to rural elders are explored.  相似文献

The authors contend that employer-initiated workplace policies should be considered as part of the patchwork that constitutes U.S. family policy. To provide a background, historical evidence of employer-initiated policies intentionally used as family policies is summarized. The view is then explicated that failure to take economic conditions and workplace policies into account may lead to faulty conclusions about the reasons for major changes in family life. The current status of selected employer-initiated workplace policies and recent trends is summarized. Next, some of the dilemmas inherent in considering connections between government- and employer-initiated policies are identified. Finally, suggestions for future directions are offered. The article should be considered with the following caveat in mind: the focus is exclusively on the influence of policies on families, but families are not merely reactive. She received her Ph.D. and M.B.A. from The Pennsylvania State University. Her research interests center on relationships between jobs and family life, with emphasis on work-family relationships in small businesses, and on adult workers as developing individuals. Her research interests focus on the intersection of social problems and family problems, including families and work, coping with unemployment, and adult children as caregivers of dependent parents. This paper presents a theory about degrees of revenge and proposes creative, libidinal, and contained ways in which to express this otherwise destructive desire. A review of the literature on revenge first underlines its inherent ubiquity, self-preservative, and adaptive propensities. The exploration of the topic draws from cultural, social/judicial, and clinical texts to elucidate the interplay of creative and destructive elements in revenge. I have used the Structural Model developed by Freud to illuminate and sharpen the argument about revenge because I feel that it is a particularly efficient lens through which to view the issue and because I regularly draw from Freudian and post-Freudian theories, amongst others, in my assessment and interventions with clients.  相似文献
