College and university compliance with a required meningococcal vaccination law

OBJECTIVE: Maryland became the first state to pass a vaccination law requiring college and university students living on campus to obtain a meningococcal vaccination or to sign a waiver refusing vaccination because college students are at increased risk for disease. The authors sought to identify how Maryland colleges addressed the law and determine whether schools were in full compliance. PARTICIPANTS: The authors surveyed 32 college/university administrators via a self-administered questionnaire. METHODS: The authors calculated vaccination and waiver rates and assessed compliance with the law overall and with specific law components. RESULTS: Among 28 participating schools, annual vaccination rates and waiver rates among students during 2000-2004 ranged from 66%-76% and 12%-17%, respectively. Two (7%) schools were compliant with all components of the law. CONCLUSIONS: Mandatory vaccination laws do not ensure compliance at the college and university level. Mandatory reporting, increased education, and collaboration between colleges and universities and public health agencies are needed.     

Understanding child neglect in Aboriginal families and communities in the context of trauma

Child neglect is a significant concern for Aboriginal families and communities both in Australia and internationally. Service responses to child neglect are largely informed by child neglect theories, which explain the nature and causes of child neglect. However, child neglect is a problem that is worsening for Aboriginal children, suggesting that these theories are not appropriate. This paper argues that to meet the needs of Aboriginal families and communities where there is child neglect, policy and practice needs to acknowledge and address the impact of trauma in shaping the lived experiences of Aboriginal people. International literature discusses the impact and consequence of historical trauma within Indigenous families and communities, and separately, child maltreatment theorists have discussed the ecological nature of child neglect. However, the literature and evidence‐base linking the two, child neglect and historical trauma in Indigenous contexts, are very scant. This paper aims to fill this gap and emphasize the importance of addressing child neglect within Aboriginal families and communities in the context of historical trauma. Although this paper focuses on the post‐colonial experiences of Aboriginal people in Australia, the arguments can extend to many Indigenous cultures universally where child welfare interventions have resulted in significant and ongoing trauma.

Problem-solving communication in foster families and birthfamilies

This study assessed child behavior problems and parent-child communication behaviors during problem solving in three groups of families with adolescent children: foster families, birthfamilies with a child at risk for behavior problems, and birthfamilies with a child not at risk. Levels of positive and negative communication behaviors in the foster families were similar to those in the lower-risk families and were significantly related to foster child behavior problems.     

Reports of childhood sexual abuse by adolescents and young adults: stability over time

The consistency of childhood sexual abuse (CSA) reporting was explored in this study. Two-hundred seventeen adolescents and young adults (ages 14-24) enrolled in urban health care clinics completed self-report questionnaires assessing CSA and other problem behaviors at enrollment and at 7 months. Results indicated that the stability of CSA self-report at two time points was poor (58% consistent nonreporters of CSA, 20% consistent reporters, 22% inconsistent reporters). Consistent and inconsistent reporters were differentiated on risk measures. Adolescents who endorsed more items from the CSA scale were five times more likely to be consistent reporters. In sum, adolescent CSA reporting was quite inconsistent over time. Using multi-item scales and assessing CSA at two time points enhances accuracy of reporting.

A deletion/substitution/addition algorithm for classification neural networks,with applications to biomedical data

Neural networks are a popular machine learning tool, particularly in applications such as protein structure prediction; however, overfitting can pose an obstacle to their effective use. Due to the large number of parameters in a typical neural network, one may obtain a network fit that perfectly predicts the learning data, yet fails to generalize to other data sets. One way of reducing the size of the parmeter space is to alter the network topology so that some edges are removed; however it is often not immediately apparent which edges should be eliminated. We propose a data-adaptive method of selecting an optimal network architecture using a deletion/substitution/addition algorithm. Results of this approach to classification are presented on simulated data and the breast cancer data of Wolberg and Mangasarian [1990. Multisurface method of pattern separation for medical diagnosis applied to breast cytology. Proc. Nat. Acad. Sci. 87, 9193–9196].
