Solving the Recruitment Crisis in UK General Practice: Time to Consider Physician Assistants?

With large numbers of GPs due to retire by 2007, the recruitment and retention crisis in UK general practice is likely to get worse before it gets better. Urgent measures are now required. Policies such as offering doctors financial incentives to enter general practice or delay retirement, while increasing the number of medical school places, may, for a variety of reasons, be likely to enjoy only limited success. In this article, a more radical solution is proposed: the creation of an intermediate medical practitioner role, along the lines of the physician assistant in the USA. The establishment of such a role may go some way to resolving the workforce crisis that currently besets general practice and ensure that medical needs, especially in deprived areas, are adequately met. A number of specific occupational groups who may find the PA role attractive are identified. It is suggested that refugee doctors, unable to practise in the UK, may find PA training particularly attractive, especially if it could serve as a stepping-stone to GMC registration.

How does information sharing affect first-tier suppliers’ flexibility? Evidence from the automotive industry in Sweden

This study addresses the critical research issue of how supplier delivery performance can be enhanced by integrating information sharing into volume and delivery flexibility. This study developed a research model to relate information sharing on demand forecasts and inventory data between original equipment manufacturers (OEMs) and first-tier suppliers. Based on a sample of 52 suppliers from automotive industry in Sweden, partial least squares structural equations modelling (PLS-SEM) was used to assess the model. The results confirm that sharing demand forecasts is a key enabler of supplier volume and delivery flexibility while sharing inventory data is not. The study contributes to enlarging the knowledge about supply chain management from the suppliers’ perspective. It also contributes to knowledge by validating the conceptual model and operationalisation of constructs. The study also has practical contribution in which management should focus on improving communication and collaboration practices with OEMs for effective sharing of demand forecasts.

Bayesian Estimation of the Change Point Using X¯ Control Chart

The process personnel always seek the opportunity to improve the processes. One of the essential steps for process improvement is to quickly recognize the starting time or the change point of a process disturbance. The proposed approach combines the X¯ control chart with the Bayesian estimation technique. We show that the control chart has some information about the change point and this information can be used to make an informative prior. Then two Bayes estimators corresponding to the informative and a non informative prior along with MLE are considered. Their efficiencies are compared through a series of simulations. The results show that the Bayes estimator with the informative prior is more accurate and more precise when the means of the process before and after the change point time are not too closed. In addition, the efficiency of the Bayes estimator with the informative prior increases as the change point goes away from the origin.
