Early Cultural Dialogues in Cross-Cultural Clinical Practice

This article examines the quality of cultural dialogues between clients and therapists when they initiate and integrate verbal references to culture in the beginning process of therapy. Six cross-cultural dyads in short-term–oriented community mental health services participated in the study. The initial three sessions of actual clinical encounters were taped, and conversation analysis was used for an intensive analysis of the treatment sessions. Despite the presence of a wide range of affectively charged cultural content raised by the clients, the therapists showed only minimal responses to these topics. The potential of therapeutically germane cultural dialogue was lost, and the discussion of material involving cultural differences regressed to monologues expressed by clients. In this sample, there was little talk that focused on cultural differences or similarities between clients and therapists. Implications and limitations of the study in cross-cultural clinical practice are discussed.     

Ego Psychological Contributions to Understanding Psychopharmacology and Clinical Practice in Social Work Education

Due to increasing evidence that supports the efficacy of combined pharmacology and psychotherapy in the treatment of multiple psychiatric disorders, an educational focus on integrated approaches is growing in social work and related fields. However, there are few guiding clinical theoretical models that can help students integrate pharmacology and clinical issues. The authors propose that ego psychology can advance clinical knowledge by helping students understand pharmacological issues and integrate pharmacotherapy with psychotherapy. Assessing ego functions in response to medication referral and use contributes to enhanced assessment and treatment.     

A modified generalized lasso algorithm to detect local spatial clusters for count data

Detecting local spatial clusters for count data is an important task in spatial epidemiology. Two broad approaches—moving window and disease mapping methods—have been suggested in some of the literature to find clusters. However, the existing methods employ somewhat arbitrarily chosen tuning parameters, and the local clustering results are sensitive to the choices. In this paper, we propose a penalized likelihood method to overcome the limitations of existing local spatial clustering approaches for count data. We start with a Poisson regression model to accommodate any type of covariates, and formulate the clustering problem as a penalized likelihood estimation problem to find change points of intercepts in two-dimensional space. The cost of developing a new algorithm is minimized by modifying an existing least absolute shrinkage and selection operator algorithm. The computational details on the modifications are shown, and the proposed method is illustrated with Seoul tuberculosis data.     

Developing a Working Model of Cross-Cultural Supervision: A Competence- and Alliance-Based Framework

Despite numerous suggestions to integrate culture, diversity and social justice issues in clinical supervision, empirical studies on cross-cultural supervision indicate limited uptake of such recommendations. We suggest that a comprehensive model of cross-cultural supervision could benefit the field by guiding supervisors in this task. A working model is proposed based on a foundation of the supervisory alliance and a focus on social work practice competence, integrating strategies to promote self- and relational-reflexivity within the supervisory relationship. The model is comprised of four components: component 1: goal setting to contract on cross-cultural integration in supervision; component 2: active listening for cross-cultural markers; component 3: bonding through the supervisor’s self-reflexivity to foster the supervisee’s self-reflexivity; and component 4: working through tasks for cultural integration in supervision by modelling the supervisor’s relational reflexivity in case formulation and treatment to foster the supervisee’s relational-reflexivity.     

A Working Model of Cross-Cultural Clinical Practice (CCCP)

A long-standing gap between clinical and cultural practice can lead clinicians to feel overwhelmed by the task of integrating clinical practice with a culturally sensitive approach, while working toward changes in clients’ lives. This article attempts to assist clinicians in their efforts to achieve this task. Using a Task-Analysis approach in the alliance research (Safran et al. 1994), this article discusses the development of Cross-Cultural Clinical Practice and proposes a detailed working model for its possible application. A case example is presented to highlight key components of the model. Limitations and implications of the model in clinical social work practice are also described.
