A comparative study conducted in the Sudan illustrated both the potential of a nutrition education program for pregnant women and the many obstacles to creating more optimal maternal nutrition. Included in the study were 1200 women 18-34 years of age attending the Khartoum Province Model Clinic for a minimum of 5 prenatal visits. 83.3% were Muslims and 48.2% were illiterate. The 60 women who comprised the experimental group were exposed to individual nutrition counseling and home visits. Overall, 70% of subjects were anemic, 27% had malaria, and 18% were hookworm. The average weight gain during pregnancy was 4.5 kgs; 25.0% of controls and 20.0% of those in the experimental group gained 3.5-4.0 kgs, 18.3% of controls and 33.3% of experimental subjects gained 4.5-5.0 kgs, and 23.3% of controls and 30.0% of those in the experimental groups gained 5.5 kgs or more. 53.0% of controls and 61.7% of women exposed to nutrition education breastfed for at least 6 months, and 70% of women in the former group and 95% of those in the latter had enrolled their infants in an immunization program. A baseline nutrition questionnaire revealed widespread misinformation, especially a belief that pregnant women should reduce their food intake from 3 to 1-2 meals per day to prevent the fetus from growing too large and making labor difficult. Foods such as fermented sorghum products, fresh fish, and water melon were regarded as abortifacients and avoided, while hot pepper, sour milk, and cola sodas were considered nutritious. Consumption was self-reported to be poor for milk products by 25% of mothers, for meat by 16%, and for fruits and vegetables by 40% Although the educational intervention increased women's knowledge of basic nutrition, it had only a minimal effect on actual practice. This finding is assumed to reflect the widespread belief that men and their guests should be given preference in food allocation, poverty, and avoidance, on the basis of tradition, of many healthy foods. Since 72% of women reported ownership of a television, use if this medium is recommended for nutrition education.  相似文献

3 groups of women are compared in this study of the effect of migration on fertility in a less developed country: 1) rural sedentary; 2) rural to rural migrants; and 3) rural to urban migrants. The data are from a 1970 household interview study conducted by the Institute of Behavioral Science, University of Colorado in Magsayay and Matanao, Davao Province, Mindanao, the Philippines. Social, economic, and mortality data were gathered from the household head and/or spouse for each household member and each child living elsewhere. Reproductive histories were obtained only from women for all women 15 years of age and older living in the 2 rural communities and living elsewhere. Age specific fertility rates and child woman ratios showed a declining gradient of fertility with social distance from the rural home communities. Age at marriage and education were positively associated with distance from the home communities and negatively associated with fertility. The data provide support for the hypothesis that recent migration is innovative, engaged in by more modernized persons who are motivated by aspiration to new goals, thus migration has a negative effect on fertility. Urbanization had its major impact after peak fertility years, 20-29, influencing urban migrants to bring their fertility under voluntary control. No such curtailment appeared in the late reproductive behavior of rural sedentary or migrant women. Urbanization seems to have a negative effect on fertility independent of migration. Young migrant women, in their teens, particularly those migrating to urban areas, did not fit the social mobility model; they tended to complete fewer years of school and married at an earlier age. These young urban migrants also had higher fertility than both rural sedentary and rural migrant females while in their teen years.  相似文献

Abstract The effects of the birth of an additional child to families living in poverty areas of New York City are studied in this paper. Surveys conducted by the National Opinion Research Center in 1965 and 1967 provided the data in a panel of parous or married women of childbearing age. Control ling for the number of children in the family in 1965, the non-occurrence of an additional birth in the following two years was found to have a significant effect on current income, savings, reliance on public assistance, general ability to plan and organize one's household, and wife's employment. No significant effects were found with respect to possession of consumer durables or attending a school or training course. While many claims have been made about the beneficial effects of family planning on family welfare, this study is among a very small number where such effects are empirically documented.  相似文献
