Several colorectal cancer (CRC) screening models have been developed describing the progression of adenomas to CRC. Currently, there is increasing evidence that serrated lesions can also develop into CRC. It is not clear whether screening tests have the same test characteristics for serrated lesions as for adenomas, but lower sensitivities have been suggested. Models that ignore this type of colorectal lesions may provide overly optimistic predictions of the screen‐induced reduction in CRC incidence. To address this issue, we have developed the Adenoma and Serrated pathway to Colorectal CAncer (ASCCA) model that includes the adenoma‐carcinoma pathway and the serrated pathway to CRC as well as characteristics of colorectal lesions. The model structure and the calibration procedure are described in detail. Calibration resulted in 19 parameter sets for the adenoma‐carcinoma pathway and 13 for the serrated pathway that match the age‐ and sex‐specific adenoma and serrated lesion prevalence in the COlonoscopy versus COlonography Screening (COCOS) trial, Dutch CRC incidence and mortality rates, and a number of other intermediate outcomes concerning characteristics of colorectal lesions. As an example, we simulated outcomes for a biennial fecal immunochemical test screening program and a hypothetical one‐time colonoscopy screening program. Inclusion of the serrated pathway influenced the predicted effectiveness of screening when serrated lesions are associated with lower screening test sensitivity or when they are not removed. To our knowledge, this is the first model that explicitly includes the serrated pathway and characteristics of colorectal lesions. It is suitable for the evaluation of the (cost)effectiveness of potential screening strategies for CRC.  相似文献

Since the 1980s outsourcing has been a major topic in both economic and management literature. Economic literature tends to focus on the transaction cost perspective. In management literature the core competence approach prevails. An emerging alternative view on outsourcing is the power and dependence perspective. Aspects of power and dependence become visible in four case studies involving the maintenance of capital assets. In each instance both the buyer and supplier sides of the dyad are examined. This study empirically investigates the safeguarding effect of (1) administrative control, (2) a dominant power position and (3) relational norms, on opportunistic behaviour of suppliers, by means of a survey among 624 information and communication technology professionals in Dutch municipalities. The findings indicate that individual effectiveness of relational norms, particularly in terms of flexibility and solidarity, was most prominent. Administrative control and power did not show a significant impact on supplier opportunism in our sample. Research into the simultaneous use of several safeguards against opportunism generated the finding that different relational norms fortify each other's safeguarding effect. Furthermore, we demonstrate that supplier opportunism is only mitigated by a dominant power position when the buyer does not exploit its favourable position and the buyer does not behave opportunistically. Hence, power seems to have a safeguarding effect only when it is not used.  相似文献

A negative effect of good health on the instrumental support received can be viewed as an effect of the mobilization of helpers. A positive effect of good health on the personal network size and the instrumental support given demonstrates that people in poor health have difficulty actively maintaining their relationships. Furthermore, the support received and given is positively related to the support given and received in the past. In four waves of a seven–year longitudinal study, personal interviews were conducted with 2,302 older Dutch adults (aged 60 to 85) who live on their own. The hypotheses have been confirmed. An implication is that investing in relationships by giving support might pay off in times of need.  相似文献

Given the growing number of elderly persons in society and concerns about their health and well-being, the aim was to review the available literature on their death anxiety, and to explore features of this experience among a small sample of older men and women in care facilities. In both the review and empirical parts of this study, components and correlates of death anxiety were investigated. The review revealed limited research focus on death anxiety among the elderly, particularly among those in institutions, but suggested both components and correlates for inclusion in our empirical study. Results showed that, among our elderly participants in an assisted living facility (N = 49; age range: 60-96 years), there were higher levels of fear for others and of the dying process than for fear of the unknown. Notably, among the correlates identified, fear for significant others was associated with poor physical health; fear of the dying process was related to low self-esteem, little purpose in life, and poor mental well-being. Gender differences in death anxiety were found: women showed greater fear for the death of loved ones and for the consequences of their own death on these loved ones, than did men. These patterns are discussed in the light of concerns about the welfare of elderly persons; scientific implications are also considered.  相似文献

Most still‐face paradigm (SFP) studies have been done in Western families with infant–mother dyads. The present study investigated the SFP pattern in 123 Dutch and 63 Chinese 4‐month‐old infants with mothers and fathers. The classic SFP effect was found for positive affect and gaze in both countries. For negative affect, Chinese infants showed a different SFP pattern than Dutch infants. With fathers, infants displayed a less pronounced SFP pattern for positive affect and an increase from the still face to the reunion for negative affect. Only a minority of infants showed the expected SFP pattern across episodes. Our findings support that infant emotion expression is influenced by parent gender and cultural context. An interesting avenue for further study is the exploration of the origins of within‐ and between‐gender and culture differences in affective communication between parents and infants.  相似文献

Research has shown that several criteria underlie people’s opinions about the welfare deservingness of benefit recipients. However, it remains unknown which factors are associated with the emphasis that people place on such criteria. Using a 2006 Dutch national survey on the welfare deservingness of disability pension recipients, we study the influence of structural and cultural factors on people’s emphasis on three deservingness criteria: control, need, and reciprocity. OLS regression analyses show that people’s emphasis on specific deservingness criteria is strengthened by structural factors that indicate the possibility of resource competition such as the following: age, lower levels of education, unemployment, and lower income. However, actual personal experience with receiving welfare benefits weakens criteria emphasis. Cultural factors such as the espousal of views from the political right and the possession of a strong work ethic are associated with a heightened emphasis on deservingness criteria.  相似文献

This is a review of published longitudinal empirical research on the impact of restructuring on employee well-being. We investigated whether restructuring accompanied by staff reductions impacts differently on worker well-being than restructuring without staff reductions, and the differences between short- and long-term effects of restructuring. Furthermore, we investigated the mechanisms that explain these effects. We conducted a literature search on longitudinal, peer-reviewed, English-written studies from the period 2000–2012. Thirty-nine papers fulfilled the inclusion criteria. We found that restructuring events, with and without staff reductions, mainly have a negative impact on the well-being of employees. The majority of studies showed negative changes over time, in the short and the long term. Some groups of workers reacted less negatively: for example, workers with a high organizational status before a merger and workers who underwent a change in workgroup. Variables that intervened in the relationship between restructuring and well-being were physical demands, job control, communication, provision of information, training, procedural justice, job insecurity and change acceptance. Further high-quality longitudinal research is needed to get more insight into the impact of restructuring over time and into the part played by intervening variables.  相似文献

Long-term trends in deservingness opinions and how these fluctuate in relation to changes in the economic, institutional and political contexts have not often been examined. In this paper, we address these trend questions by analyzing 22 waves of the repeated cross-sectional Cultural Change in The Netherlands (CCN, 1975–2006) survey. Our analyses show fairly stable public deservingness opinions regarding five different needy groups over the long term. Over the short term, opinions fluctuate more. Explanatory analyses show that economic and political factors, but not institutional factors, are especially influential over fluctuations in opinions. When real GDP grows, the Dutch public is more likely to consider the disabled, the elderly and social assistance beneficiaries deserving of more welfare support. In addition, when unemployment rises, the unemployed and social assistance beneficiaries are more likely to be seen as deserving of more support. Finally, when the national political climate is more leftist, most needy groups are considered to be deserving of more welfare support.  相似文献
