As STD infections including HIV increase in the United States, it has become increasingly important to policy makers to ascertain the extent to which knowledge and perceptions of AIDS risk affect an individual's probability of altering their sexual or contraceptive behavior to avoid infection. This paper examines the extent to which women's perceptions of their own and their partners' risk of HIV infection affects the probability of using a condom for protection against sexually transmitted diseases. This paper also examines the extent to which HIV testing may affect motivation for condom use. Crosstabulations reveal that prophylactic condom use is more prevalent among women who have been tested for HIV and increase as perceptions of their chance (and their partner's chance) of being positive increases. The multivariate results from this study indicate that having an HIV test significantly predicts the likelihood of using a condom for STD prevention for US women. Furthermore, women who perceive themselves to be at least somewhat likely to be HIV positive have a higher probability of using a condom to prevent sexually transmitted disease, and women who perceive their partners to be HIV positive are twice as likely as other women to use a condom for STD prevention. However, women who consider themselves likely to be HIV positive are no more likely to use a condom than those who consider themselves not at risk.

ABSTRACT

The purpose of this study was to describe sexual behaviors that youth substitute for protected intercourse when a partner refuses to use a condom or a condom is not used. Participants included: (a) 120 adolescents (mean age 17.2 years) undergoing substance abuse treatment and (b) 171 university students (mean age 21.3 years). The treatment sample reported significantly higher levels of discrete risk factors for HIV exposure. While only a small proportion of each sample experienced outright refusals by partners to use condoms, the majority of participants in each sample used condoms inconsistently with main partners. The university sample reported more substituted behaviors (lower risk and total) when partners did not want to use condoms. Yet, substantial proportions of participants in each sample engaged in unprotected intercourse. Implications for social work and health promotion efforts directed toward youth at risk for HIV/STD exposures are discussed.

With the growing number of sexually transmitted infections (STIs) among young people (15–24) in the US, condom distribution programs (CDP) remain an integral part of prevention strategies. The objective of the study was to gather qualitative insights from CDP planners to inform effective CDP implementation. Ten semi-structured interviews with program planners were conducted via telephone (response rate of 58.8%). Condom distribution channels included site-based distribution (n = 6), web-based distribution (n = 4), and distribution via Uber (n = 1). Site-based distribution programs had distribution networks ranging from 100 to 3500 sites in a given jurisdiction. Planners of site-based CDPs experienced challenges with ensuring quality control at sites and supplying condoms to sites. Web-based CDPs allow individuals to order free condoms online. These CDP planners reported growing demand for this discreet service, particularly among young people. Web-based CDPs leveraged e-mail surveys to collect data on sexual behavior and behavior change, yielding response rates as high as 63%. All CDPs emphasized the importance of supplying a variety of products. Total supplies distributed (male condoms, internal condoms, lubricant sachets) ranged from 16,000 to 45 million per year. Novel channels of distribution should be explored to ensure reach to adolescents and young adults.

This study examined the impact of risk factors, self and peers’ beliefs, and parental communication on condom use among 290 female adolescents who attended family planning clinics. The major findings showed that in addition to risk factors, self and peer beliefs about condom use and parental communication about birth control predicted actual condom use. Implications for social workers about the importance of encouraging communication between adolescents and their parents are discussed.

ABSTRACT

Despite knowledge of the effectiveness of condoms in preventing HIV transmission, condoms continue to be reported in only a minority of sexual encounters even with nonpermanent partners. This study used focus-group discussions with youth and interviews with community leaders from 22 communities in the Nyanza and Rift Valley provinces, Kenya, to examine the sociocultural influences on condom use among school-going youth. Three overarching themes emerged from a thematic analysis of the data. Condoms did not fit with the purposes, meanings, and understandings of sexuality; in fact, condom use violated most of these. Condoms were also thought to be dangerous for girls and women, to contribute to the spread of HIV, and to be unnecessary for protection against HIV. Finally, given the hardships of life and multiple diseases and circumstances that threatened life, HIV was seen as just another trouble of living. Sex without a condom, within this context, was described as one of the pleasures of a short life. The complexities of sexuality evidenced in the discussions of these youth and adults challenge the viability of the ABC (abstain, be faithful, use a condom) message as a preventive measure against HIV transmission.

This study examined the influences of behavior change communication (BCC) campaigns on knowledge and use of condoms for prevention of HIV/AIDS and other sexually transmitted infections in target areas of Uganda. Data were drawn from the 1997 and 1999 Delivery of Improved Services for Health (DISH) evaluation surveys, which collected information from representative samples of women and men of reproductive age in the districts served by the DISH project. Logistic regressions were used to assess the associations between BCC exposure and condom knowledge and use, controlling for individuals' background characteristics. Results indicated that BCC exposure was strongly associated with higher condom knowledge: women and men who reported being exposed to messages in the mass media were at least twice as likely as those with no exposure to know of condoms as a mean to avoid HIV/AIDS. A dose–response effect was observed, suggesting that campaigns using multiple media channels may be most effective in improving sexual health knowledge. At the same time, certain gender differences of the influences on condom use were found, with the channel and content of messages seemingly more important in terms of instilling safer sex practices. While there was some evidence of bias of self-report, exploratory analysis of the indirect effects of communications campaigns suggested that impacts may be compounded as overall awareness is increased at the community level eventually leading to behavior change among individuals.

Cross-over trials with correlated Bernoulli outcomes are common designs. In condom functionality studies, for example, an indicator of condom failure is reported for each sex act using standard or experimental condoms. Two popular analysis methods for such data are Generalized Estimating Equations and logit-normal random effects models. An alternative random effects model, the beta-binomial, is commonly used in contexts involving only between-cluster effects. The flexibility of the beta distribution and the interpretation of random effects as cluster-specific failure probabilities make it appealing, and we consider an extension of the model to account for within-cluster treatment effects using proportional odds assumptions.

Objectives: The objective of this study was to investigate factors of condom use among a sample of 490 sexually active Chinese college students. Methods: A cross-sectional survey by systematic sampling was conducted on students' interpersonal sexual communication, media exposure, and condom use. Multiple regression was employed for analysis. Results: Self-efficacy, peer and partner sexual communication, parent norms about condom usage, HIV/AIDS information from brochures, and exposure to sexual content in books were associated with variables related to condom use. Conclusions: The current study underscores the importance of self-efficacy and peer and partner sexual communication in HIV preventive intervention programs that promote condom use among young Chinese.
