We describe a risk-based analytical framework for estimating traffic fatalities that combines the probability of a crash and the probability of fatality in the event of a crash. As an illustrative application, we use the methodology to explore the role of vehicle mix and vehicle prevalence on long-run fatality trends for a range of transportation growth scenarios that may be relevant to developing societies. We assume crash rates between different road users are proportional to their roadway use and estimate case fatality ratios (CFRs) for the different vehicle-vehicle and vehicle-pedestrian combinations. We find that in the absence of road safety interventions, the historical trend of initially rising and then falling fatalities observed in industrialized nations occurred only if motorization was through car ownership. In all other cases studied (scenarios dominated by scooter use, bus use, and mixed use), traffic fatalities rose monotonically. Fatalities per vehicle had a falling trend similar to that observed in historical data from industrialized nations. Regional adaptations of the model validated with local data can be used to evaluate the impacts of transportation planning and safety interventions, such as helmets, seat belts, and enforcement of traffic laws, on traffic fatalities.  相似文献

Research on the risk of motor vehicle injuries and their relationship with the amount of travel has been only partially analyzed. The few individual exposure assessments are related to very specific subsets of the driving and traveling populations. This study analyzes the relationship between kilometers traveled and hospitalization due to motor vehicle injuries. Twelve thousand three hundred and sixty nine Spanish university graduates from the Seguimiento Universidad de Navarra multipurpose cohort study were evaluated. They had not been hospitalized due to motor vehicle injuries at baseline and were followed up to eight years. Biannual questionnaires allowed for self‐reporting of kilometers traveled in motor vehicles, together with incidence of hospitalization. Covariates in the Cox regression models included age and gender and baseline use of safety belt while driving, driving a vehicle with driver‐side airbag, driving a motorcycle, and drinking and driving. There were 49,766 participant‐years with an average yearly travel of 7,828 km per person‐year. Thirty‐six subjects reported a first hospitalization event during this time. The adjusted hazard ratio per additional kilometer traveled was 1.00005 (95% confidence interval 1.000013 to 1.000086). Even the smallest of reductions in the amount of kilometers traveled (from an average of 3,250 km per year to 1,000) has a statistically significant protective effect on the likelihood of sustaining hospitalization due to motor vehicle injury (aHR 0.9, 95% CI 0.78 to 0.98). In light of current policies aimed to reduce motorized traffic due to environmental concerns, it may be appropriate to consider the additional health benefit related to reductions in injuries.  相似文献

Occupational accidents and injuries continue to be a critical concern for nurses, given the hazardous healthcare environment. This study advances the research on workplace safety by studying the process variables (i.e. job-related negative affect (JRNA) and job satisfaction) in explaining the relationship between safety climate and various safety criteria in nurses. Based on survey data from 326 nurses, our findings suggest that psychological safety climate is negatively related to JRNA, turnover intentions, safety workarounds, and workplace hazards. In addition, structural equation modelling indicated general support for a model in which psychological safety climate influences employee strain through job attitudes, including JRNA and job satisfaction. More specifically, job attitudes were found to mediate the relationship between psychological safety climate and turnover intentions, experience of hazards, and injuries. Safety workarounds did not significantly relate to injuries. The present study contributes to the ongoing improvement of interventions aimed at mitigating nurses’ injuries by integrating job attitudes into the safety climate–safety outcome framework.  相似文献

Bayesian networks (BNs) are probabilistic expert systems which have emerged over the last few decades as a powerful data mining technique. Also, BNs have become especially popular in biomedical applications where they have been used for diagnosing diseases and studying complex cellular networks, among many other applications. In this study, we built a BN in a fully automated way in order to analyse data regarding injuries due to the inhalation, ingestion and aspiration of foreign bodies (FBs) in children. Then, a sensitivity analysis was carried out to characterize the uncertainty associated with the model. While other studies focused on characteristics such as shape, consistency and dimensions of the FBs which caused injuries, we propose an integrated environment which makes the relationships among the factors underlying the problem clear. The advantage of this approach is that it gives a picture of the influence of critical factors on the injury severity and allows for the comparison of the effect of different FB characteristics (volume, FB type, shape and consistency) and children's features (age and gender) on the risk of experiencing a hospitalization. The rates it consents to calculate provide a more rational basis for promoting care-givers’ education of the most influential risk factors regarding the adverse outcomes.  相似文献

This paper estimates both single and multiple spell hazard models using a sample of workers' compensation claims (with soft tissue musculoskeletal injuries) from Ontario, Canada. Most of the results in this paper are consistent with the estimates in the existing literature. However, the duration elasticities for workers' compensation benefits and pre-injury earnings from the multiple spell hazard models are much larger than the estimates from the single spell models. This suggests that the estimates in existing literature that use single spell hazard models may underestimate the effect of benefits and pre-injury earnings on the duration of soft tissue musculoskeletal claims.  相似文献

Sport injuries restrict participation, impose a substantial economic burden, and can have persisting adverse effects on health‐related quality of life. The effective use of Internet of Things (IoT), when combined with analytics approaches, can improve player safety through identification of injury risk factors that can be addressed by targeted risk reduction training activities. Use of IoT devices can facilitate highly efficient quantification of relevant functional capabilities prior to sport participation, which could substantially advance the prevailing sport injury management paradigm. This study introduces a framework for using sensor‐derived IoT data to supplement other data for objective estimation of each individual college football player's level of injury risk, which is an approach to injury prevention that has not been previously reported. A cohort of 45 NCAA Division I‐FCS college players provided data in the form of self‐ratings of persisting effects of previous injuries and single‐leg postural stability test. Instantaneous change in body mass acceleration (jerk) during the test was quantified by a smartphone accelerometer, with data wirelessly transmitted to a secure cloud server. Injuries sustained from the beginning of practice sessions until the end of the 13‐game season were documented, along with the number of games played by each athlete over the course of a 13‐game season. Results demonstrate a strong prediction model. Our approach may have strong relevance to the estimation of injury risk for other physically demanding activities. Clearly, there is great potential for improvement of injury prevention initiatives through identification of individual athletes who possess elevated injury risk and targeted interventions.  相似文献

In this article, we examine how mothers respond when injury interrupts maternal care, using the lens of embodied care, which we conceptualize as a form of ‘body work’. We draw on findings from a qualitative research project with two organizations in Australia that help people with injuries to return to work, examining the experiences of workers who are also mothers of dependent children. Mothers' inability to care for children during periods of injury was a significant concern for our interviewees; constraints on physical labour and physical affection were particularly troubling, indicating the importance of embodied maternal caregiving to maternal roles. Yet, while these mothers inhabited the spheres of paid work and unpaid care work simultaneously, service providers did not consider embodied care work or its relevance to injured women's ongoing needs for support. While our findings reflect the experiences of injured women, they also suggest the need for a materialist analysis of the ways that both paid work and care activities are deeply enmeshed in and through the bodies of those doing the work. Employers and service organizations still fail to recognize maternal ‘body work’, and this may be typical of social attitudes more widely.  相似文献

Count data series with extra zeros relative to a Poisson distribution are common in many biomedical applications. A score test is presented to assess whether the zero-inflation problem is significant to warrant the analysis by the more complex zero-inflated Poisson autoregression model. The score test is implemented as a computer program in the Splus platform. For illustration, the test procedure is applied to a workplace injury series where many zero counts are observed due to the heterogeneity in injury risk and the dynamic population involved.  相似文献
