Predictably, certain patients present treatment issues which resonate strongly with issues that are still problematic (unconscious and/or only partially resolved) for the therapist. This paper examines the author's professional/personal development over forty years, beginning with reflections about a countertransference enactment triggered by an acting-out African American child whose father, his primary caregiver, had abandoned him at age 18 months. Happy or disturbing events may occur in a therapist's life during the course of treatment that intrude on the therapeutic process whether or not their true nature is disclosed to the patient. Therapists are not immune from experiencing acute, chronic, and even terminal illnesses, divorce, remarriage, adopting a child, mourning the death of a parent or significant other, or major accomplishments. In many instances such events affect the treatment process by disrupting appointments, necessitating sudden absences, restricting a therapist's emotional availability and physical stamina, or altering the therapist's mood and affect. In other instances, patients may be aware of such events, at least unconsciously, because of subtle changes in the therapist. Drawing on self psychology and intersubjectivity this paper explores the reasons for therapist self-disclosure of these events based on an assessment of the patient's developmental needs and the nature of the transference. It will discuss ways of making such disclosures therapeutic.  相似文献

Abstract

Trauma therapy influences the personal and professional lives of therapists as they cope with the secondary traumatic stress associated with treating trauma survivors. Therapists go through an internal process as they try both to make sense out of the stories they hear from clients, and to integrate those stories into their own existing cognitive schemas. During this process of integration, trauma therapists often experience secondary traumatic stress reactions that negatively impact the treatment process, as well as their own experiences of self. Secondary traumatic stress, vicarious traumitization, and worker burnout are distinctly different processes that practitioners need to differentiate, even though they are often similar in their initial presentation.  相似文献

Being a cross‐cultural systemic therapist, clinical supervisor, and educator means that culture and language are central to my work. They provide a scaffold to develop deeper understanding, increased trust, and connection between myself and my supervisees, students, and clients and facilitate a process for the latter to connect to their own selves and values. Given the fear and uncertainty generated by the COVID‐19 pandemic, there exists a pervasive activation of the sympathetic nervous system in the community. In this article, I present two case studies as examples of a cross‐cultural/cross‐linguistic approach that facilitates two clients to find a place of comfort and calmness and consequently a balancing activation of the para‐sympathetic nervous system. First is a client who, due to the overwhelming pandemic chaos, suddenly exhibited a host of signs and symptoms of a functional neurological nature, which she experienced as a lack of control and disconnection from her body, her primary language, and herself. Within a trusted therapeutic relationship that draws on the culture and primary language of the therapist, the client regains connection with language and enhances her ability to communicate and connect with her body. Second is an international student who is encouraged to use her culture of origin and primary language to induce calmness, reconnect with herself, and return to the familiar as a ‘known’ collectively inclusive, comforting, and nurturing environment.  相似文献

The death of one's therapist can be considered a traumatic event for the patient. The closeness of the relationship between therapist and patient, especially if the relationship is long-term, makes the loss analogous to losing an intimate attachment. This article will explore the complexity of issues involved for the patient who loses a therapist due to sudden death and examines the issues for the inheriting therapist as well. Case examples will be used to highlight some of the more stressful areas of practice in this difficult transition.  相似文献

This study represented a 10-year review of 86 master's theses and doctoral dissertations on play therapy that had been conducted in Taiwan from 2002 to 2011. Particular attention was directed to the intention of research, the traits of participants, the characteristics of play therapy, and the research methods. Results indicated that the most frequently studied population was children and adolescents (54.6%), followed by play therapists (25.6%), parents/care-givers of children (16.3%), and teachers of children (1.2%). Findings revealed the types of research were qualitative (n = 59; 68.6%), quantitative (n = 7; 8.1%), and mixed methods (n = 20; 23.3%). Results are discussed concerning implications for counselor educators and further research on play therapy.  相似文献

The COVID‐19 pandemic has changed the delivery of clinical services and education of health professionals, including family therapists. This paper distils two separate Zoom conversations between myself (as the lead author) and two eminent family therapists, Professors Maurizio Andolfi and Harry Aponte, where challenges and opportunities for the profession during and after the pandemic are discussed. Creativity and resourcefulness are two important elements therapists and educators have needed to access during the pandemic to find alternative ways to continue to provide clinical services and teaching. Most therapists have transitioned using online technology and various platforms such as Zoom and Skype; for some this has been a somewhat familiar experience, for most it has been a novel one. Key themes emerged from the conversations including the personal and professional ‘lived experiences’ of the pandemic; the financial impact on clients and students; the importance of touch for human social connection; the use of ‘self’ as an instrument of change and alternative platforms of service delivery and teaching. We reflected on what has been lost, such as the nuances inherent in face‐to‐face human interactions, and what has been gained, such as observing families in situ in their own environments.  相似文献

The major issue for this research was to investigate the extent and rate of divorce, its causes and attitudes toward divorce laws. This study surveyed 3500 people from the selected local government areas. The findings show high divorce rate, and 71.2% of the husbands had their divorce in courts as opposed lo 76.6% of the wives who had their marriages annulled in courts. Adultery ranked the highest among the reasons for divorce, while the wives were more likely than their husbands to have negative attitudes toward divorce laws because they are always the victims of divorce not protected by the law.  相似文献

This article examines the concept of the difficult patient from a two person perspective. Patient characteristics and behaviors may be problematic but the degree of difficulty is also related to therapist expectations, affective responses and needs, and capacity for tolerance. Suggestions for enhancing tolerance are offered. The shift from a one person to a two person perspective is illustrated by a case presentation.  相似文献
