糖尿病高血糖危象临床治疗体会及分析

目的探讨分析糖尿病高血糖危象的治疗方法和临床疗效。方法回顾分析我院2013年4月至2013年12月治疗的80例糖尿病高血糖危象患者的临床资料，采用随机数字表法分为2组，各40例，两组均充分补液、纠正电角质失调，根据患者情况采取补钾、适量补碱治疗，同时积极寻找和消除诱因，防治并发症，在胰岛素治疗及纠正水电解质酸碱紊乱、防治并发症方面，A组采用胰岛素泵连续皮下输入胰岛素治疗及小剂量胰岛素静滴，部分患者配合血液净化治疗；B组采用静脉滴注小剂量胰岛素及餐前皮下注射胰岛素治疗，对比其疗效。结果两组血糖均显著降低，前后差异具有统计学意义（P〈0．01），A组平均血糖低于B组，差异具有统计学意义（P〈0．01），A组出现低血糖概率大于B组，但A组发生血糖反复概率较少，纠正水电解质酸碱紊乱效果较好，并发症少，差异显著（P〈0．01）。结论在补液、补充电解质及小剂量胰岛素治疗，配合血液净化、消除诱因可有效降低糖尿病患者血糖，使其趋于稳定，减少并发症发生，降低病死率。

Treatment experiences of hyperglycemic crisis in diabetes patients

Objective To discuss and analyze the treatment methods and clinical efficacy of hyperglycemic crisis in diabetes pa- tients. Methods Retrospectively analyzed 80 cases hyperglycemic crisis diabetes patients＇ clinical date from April 2013 to February 2013, and randomized by random digits table into two groups, each group has 40 cases, both groups conventionally used fluid infu- sion, electrolyte replacement, and used potassium supplement, alkali supplement based on patients＇ specific circumstances. In terms of insulin therapy, A group used insulin pump constantly infused insulin subcutaneous, B group used intravenous infusioa of insulin. Compared their clinical efficacy. Results Both groups blood sugar are significantly decreased, and the differences aft~ r and before treatment have statistically significant （ P 〈 0. 01 ）. A group＇s average blood sugar were lower than B group, but the prob ibility of hy- poglycemia in A group was higher than B group, the differences have statistically significant （ P 〈 0. 01 ）. Conclusion Use fluid in- fusion,electrolyte replacement and insulin therapy can effectively reduce diabetes patients＇ blood sugar, and make it become more stable.
