| 摘 要： | 目的探讨TCD与SPECT在老年人脑梗死中临床特点，缺血改变，敏感性及特异性。方法212例分为无症状脑梗死（SCI）及有症状及脑梗死（CI），二者在TCD与SPETC中缺血的阳性率，MRI分度的轻重与TCD和SPETC中缺血的阳性的关系。结果二者中TCD检出缺血阳性率高于SPECT，而有症状组均明显高于无症状组（P〈0．05）。TCD表现有缺血的患者中仅40％在SPECT上有缺血表现。两种方法的阳性率与MRI上反应的病情轻重呈正比。TCD和SPETC均能反应病情的轻重。TCD敏感性高于SPECT，特异性低于SPECT。结论SCI组在TCD及SPECT上仅有少数表现出缺血，而CI组中缺血的程度及范围均明显高于SCI组。MRI用于诊断。TCD用于初诊，价廉，方便。SPETC可明确梗死后缺血的轻重及范围。

| Abstract: | Objective To investigate the clinical characteristics,ischemia, sensitivity and speciality in elderly patients with cerebral infarction by TCD and SPECT examination. Methods 212 patients were divided into silent cerebral infarction (SCI) and cerebral infarction (CI),and compared inside contact between the ischemic positive rate and sever degree on MRI abnormality.
