老年冠心病患者红细胞免疫功能与BNP、CD62P的临床意义

目的探讨老年冠心病（CHD）患者外周血红细胞免疫功能（EIF）与脑钠肽（BNP））、P-选择素（CD62P）的变化及其临床意义。方法采用“郭峰改良法”、干式快速荧光免疫法及流式细胞技术检测88例老年CHD患者外周血红细胞C3b受体花环率（RBC—C3bRR）、红细胞免疫复合花环率（RBC—ICR）、BNP和CD62P，并与32例健康老年人比较。结果CHD组的RBC—C3bRR与对照组无明显差异，RBC—ICR、BNP及CD62P则CHD组明显高于对照组，尤以急性心肌梗死组（AMI）和不稳定型心绞痛组（UAP）为甚（P〈0．01）。结论老年CHD存在严重的EIF低下，免疫机制和炎症因子BNP、CD62P可能是急性冠脉综合症（ACS）发病中的一个重要因素，并可能与斑块的不稳定性有关。

The change of erythrocyte immune function, BNP and CD62P in elderly patients with coronary heart disease

Objective To investigate the changes of the peripheral erythrocyte immune function （EIF）,brain natriuretic peptide （BNP）,and P-selectin （CD62P） in elderly patients with coronary heart disease （CHD） and to discuss their clinical significances. Methods RBCs from peripheral blood of 88 elderly patients with CHD are tested for the rates of RBC- C3b rosette receptor （RBC-C3bRR）,RBC immune complex rosette （RBC-ICR）,BNP and CD62P by using Guo Feng＇s revised method,fast fluoroimmunoassay,and flow cytometry techmique. 32 healthy elders are served as control. Results No apparent difference in RBC-C3bRR is observed between the CHD group and the control group. However,RBC-ICR,BNP and CD62P are apparently higher in the CHD group,especially for elderly patients with acute myocardial infarction （AMI） and unstable angina pectoris （UAP） （P〈0.01）,than those in the control group. Conclusions There is serious impairment of EIF in elderly patients with CHD. The immune mechanisms and inflammation factors BNP and CD62P may be one of the important factors in the pathogenesis of acute coronary syndrome （ACS）, and they are possibly related to the instability of plaque.
