论基本医疗有限责任与医疗保障无限需求的矛盾

我国现行的城镇职工基本医疗保险制度自颁布实施以来,对保障参保职工的基本医疗需求发挥了重要作用,但它是以"低水平"、"广覆盖"为原则建立起来的,只能提供基本医疗,承担有限责任,而不同收入状况和不同健康状况的人群对医疗保障的需求是有差别的,且人们的医疗需求是不断增长的、无限的。这样,在基本医疗的有限责任和医疗保障的无限需求之间便产生了矛盾,这一矛盾的解决仅仅依靠基本医疗保险这种单一的保障层次是难以实现的。因此,以多层次城镇医疗保险填补基本医疗保险的责任空缺,是完善我国医疗保险制度的必要手段和有效措施,也是深化医疗保险制度改革的现实要求和必然取向。

On the Contradictions between the Limited Liability of the Basic Medicare and the Unlimited Demand in Medical Insurance

Since the promulgation and enforcement of the Basic Medical Insurance System for Urban Workers in China, it has played a vital role in ensuring the basic medical demand of the insurants. However, as it is based on the principle of"low level"and"wide coverage",it merely provides service of basic medical insurance and takes limited liability, while people with different income and health conditions may have different requirements for medical security. Meanwhile, people's demand for medical service is continuously increasing. Thus, there occurs contradiction between the limited liability of basic medical service and the unlimited demand of medical security. It is difficult to solve this inconsistency only depending on the unitary security level of basic medical insurance. Therefore, it is a necessary means and effective measure to fill the liability vacancy in basic medical insurance on the basis of multi-level urban medical security. It's also a practical requirement and unavoidable trend to deepen the reform of medical security system.
