无创双水平正压通气治疗老年心源性急性肺水肿的疗效观察

摘    要:目的 评价无创双水平气道正压通气(BiPAP)治疗老年心源性急性肺水肿的疗效及安全性。方法 对40例老年心源性急性肺水肿患者随机分组,治疗组常规药物治疗加BiPAP面罩ST-D-30呼吸机正压通气,对照组常规药物治疗加鼻导管吸氧。结果 治疗组无创面罩通气后,19例在30min~2小时症状好转,呼吸减慢,心率下降,PaO2上升,PaCO2下降,治疗总有效率95%,对照组在相应时间内仅14例好转,治疗总有效率70%。两组相比差异显著(P〈0.05)。结论 BiPAP治疗老年心源性急性肺水肿效果显著,安全可行。

Abstract:Objective To estimate the efficacy and safety of Bi-level positive airway pressure ventilation in the treatment of the elderly patients with acute cardiogenic pulmonary edema. Methods Forty aged patients with acute cardiogenic pulmonary edema were randomly divided into two groups. Therapy group were treated with BiPAP ST-D-30 mechanical ventilation by face mask in addition to routine drug,and control group were treated with routine drug and sucking oxygen through nose pipe. Results For therapy group,after 30min~2hours of non-invasive pressure support ventilation,the clinical symptoms of 19 cases were improved significantly,and respiratory rate and heart rate became slow,PaO2 was higher and PaCO2 was lower. The differences were significant(P〈0.05). Efficiency rate was 95%. In control group there were 14 cases improved in the same time,efficiency rate was 70%. There were significant difference between the two groups(P〈0.05). Conclusion It is effective and safe to treat elderly patients with APE by BiPAP.
