慢性阻塞性肺疾病患者肺通气功能检查结果分析

目的探讨肺功能检测中通气各项指标在高龄慢性阻塞性肺疾病（COPD）诊断及病情评估中的意义。方法对我科2007年11月至2008年11月200例70岁以上COPD患者肺功能检查中通气指标进行回顾性分析．并使用SPSS13．0软件对肺功能各项指标进行分析。结果COPD轻、中、重、极重度患者的FEV1、FVC、FEV1／FVC、PEF、MMEF、V50V25MVV与正常对照组均有下降（P〈0．001）；COPD各个组间FEV1均有统计学差异（P〈0．001）；COPD轻度患者的MMEF、V50、V25与正常对照组相比即有明显下降（P〈0．001）；中度、重度及极重度COPD患者的VC与其他各组相比有明显下降（P〈0．001）；各组MVV、PEF与FEV，均有高度正相关性。结论肺通气功能检查是诊断COPD的金标准，MMEF、V50、V25为检测早期COPD患者的敏感指标；FEV1、PEF、MVV均是反映COPD气流阻塞程度的指标。

The analysis of lung function of patients with chronic obstructive pulmonary disease

Objective To analyse the lung function parameters of the elderly patients with chronic obstructive pulmonary disease （COPD） to explore the clinical significance of lung function in the diagnosis and evalution of COPD. Methods The lung function of 200 patients with was retrospectively analysed by using SPSS 13.0 software. Results FVC, FEV1.FEV/FVC.PEF.MMEF.V50.V25 and MVV decreased in patients with grade Ⅰ,Ⅱ,Ⅲ and Ⅳ compared with the num ber （P〈0.001）. VC decreaedin patients with grade m and Ⅳ compared with grade Ⅰ,Ⅱ and the number （P〈0.001）. There was a positive relationship between MVV,PEF and FEV1. Conclusion Lung function is the golden standard in the diagnosis of COPD. MMEF.V50 and V25 obviously change in the early stage of COPD,FEVx,PEF and MVV are indices which reflect the obstruction degree of COPD.
