社区居民家庭医生签约意愿影响因素实证研究

以全科医学服务为基础的家庭医生制度是解决我国目前城市初级医疗保健供给的核心机制。在对南京市529名社区居民签约意愿及其影响因素调研的基础上，发现可以从以市场需求为导向设置家庭医生核心服务内容、从严格全科医生考核审批机制和准入机制角度提高家庭医生的职业素养、参考内部市场制建立家庭医生高效服务的内在激励机制、完善首诊转诊制及家庭医生培养机制及就业激励机制等方面进行制度优化，以促进居民签约。

Empirical Study on Factors of the Willingness of Signed Family Doctors in Community Residents

Family doctor system with general medical services is the core mechanism essential to the solution of current city primary supply of medical care in china.Based on investigation of Nanjing 529 community residents＇ in-tention for signing family doctors and its possible factors,it can be found that the family doctor service contents can be set from the market-oriented demand.Family doctors＇ professional accomplishment should be improved by strict criteria,approval mechanism and access mechanism for a family doctor.Internal market system should be referred to establish a intrinsic motivational mechanism for efficient service of family doctors and the first treatment mecha-nism,family doctor training mechanism and employment incentive mechanism and other aspects of system optimiza-tion should also be improved in order to promote willingness of residents for a signed family doctor.
