Poor quality of care may have a detrimental effect on access and take-up and can become a serious barrier to the universality of health services. This consideration is of particular interest in view of the fact that health systems in many countries must address a growing public-sector deficit and respond to increasing pressures due to COVID-19 and aging population, among other factors. In line with a rapidly emerging literature, we focus on patient satisfaction as a proxy for quality of health care. Drawing on rich longitudinal and cross-sectional data for Spain and multilevel estimation techniques, we show that in addition to individual level differences, policy levers (such as public health spending and the patient-doctor ratio, in particular) exert a considerable influence on the quality of a health care system. Our results suggest that policymakers seeking to enhance the quality of care should be cautious when compromising the level of health resources, and in particular, health personnel, as a response to economic downturns in a sector that traditionally had insufficient human resources in many countries, which have become even more evident in the light of the current health crisis. Additionally, we provide evidence that the increasing reliance on the private health sector may be indicative of inefficiencies in the public system and/or the existence of features of private insurance which are deemed important by patients.  相似文献

BackgroundHaving a positive childbirth experience is an increasingly valued outcome. Few studies evaluated the women’s satisfaction with childbirth through face-to-face interviews out of the health service environment. The objective of this study was to identify factors associated with a higher level of satisfaction with the childbirth experience among Brazilian women.MethodsThis cross-sectional study involved 287 women giving birth in two hospitals in southern Brazil. Women who gave birth to healthy newborns at term were randomly selected. Face-to-face interviews were conducted 31–37 days after delivery, at the mothers’ homes, using a structured questionnaire. Satisfaction with the childbirth experience was measured using a Likert-type scale ranging from very satisfied to very dissatisfied. Prevalence ratios (PR) were estimated using Poisson regression with robust variance.ResultsFollowing hierarchical multivariate analysis, the following factors remained associated with a higher level of satisfaction with the childbirth experience: being satisfied with antenatal care (PR = 1.30; 95% confidence interval [95%CI] = 1.06−1.59), understanding the information provided by health professionals during labor and delivery (PR = 1.40; 95%CI = 1.01−1.95), not having reported disrespect and abuse (PR = 1.53; 95%CI = 1.01−2.31), and having had the baby put to the breast within the first hour of life (PR = 1.63; 95%CI = 1.26−2.11). No association was observed with type of delivery or hospital status (public or private).ConclusionsA higher level of satisfaction with the childbirth experience is related to satisfactory antenatal care, a non-abusive, respectful, and informative environment during childbirth, and to the opportunity to breastfeed the baby within the first hour of life. In clinical practice, greater attention to these basic principles of care during pregnancy and delivery could provide more positive experiences during birth.  相似文献

随着现代教育的发展,提升幼儿园教师专业素养已成为促进学前教育发展的必然要求。以探究民办幼儿园教师专业素养为视角,提出通过干预行为提升教师专业素养水平的构想。实验随机选取成都市某民办幼儿园的20名教师为实验组,再选取与其办园环境、办园背景相似的另一所民办幼儿园的20名教师为对照组,对实验园教师的专业理念与师德、专业知识、专业能力3个维度进行干预。实验结果表明:与对照园相比,实验园教师的专业能力提升显著;与实验园实验前后相比,教师在专业知识、专业能力维度及专业素养总体水平方面均有提升。对教师培训的具体建议为:(1)单一与混合相结合的培训方式对提升民办幼儿园教师专业素养总体水平有现实意义;(2)民办幼儿园教师专业知识的提升需理论培训与自主学习相结合;(3)专家"现场诊断-指导式"培训方式对提升民办幼儿园教师专业能力有显著作用。综上,建议民办幼儿园在了解本园教师专业素养水平的基础上,针对教师专业素养的不同方面,分别采取不同的有效培训方式。  相似文献

Drawing on Delgado and Yosso’s counterstory, Yosso’s community cultural wealth, and Alsup’s borderland discourses, the authors, who are women of color academics, use narratives from their lives to discuss the ways in which they draw on resources in managing and reconfiguring their multiple identities within the academy. These include identities of scholars, mentors, teachers, community members, mothers, and partners. They suggest that rather than merely being socialized into cultural reproduction, as much of the literature oriented toward women of color advises them to do in order to become successful, they seek to actually engage in transforming their roles and that of the academy by consciously and repeatedly making present and visible facets of identity that have previously been more-or-less absent in higher education. By presenting these counter-narratives the authors attempt to engage with ways of self-positioning that are, especially for women of color in academia, not frequently discussed or presented.  相似文献

In order to meet the significant staffing challenges to health care provision in South Africa, the need to support the training and retention of Black medical specialists has been identified. This small qualitative study aimed to explore experiences of mentorship among a group of Black medical specialists in hopes of understanding the value and potential challenges that these relationships raise. Ten Black medical specialists were interviewed about their experiences of being mentored during their registrar training. The interview data was analysed using discourse analysis, and the results comprise of three main themes, namely: Difficulty accessing mentorship, a lack of Black and/or female role models and finally, experiences of feeling excluded from soft networks. The experiences explored in this study indicate the need for open dialogue within the mentorship dyad regarding certain taken-for-granted assumptions surrounding race, so as to aid the provision of more effective and satisfying mentorship outcomes.  相似文献

The goal of this study is to provide a cross-lagged examination of the relationships between engaging leadership, job resources and employee work engagement. We propose a mediation model and we postulate that engaging leadership can increase perceptions of three specific job resources (i.e. autonomy, support from colleagues and opportunities for learning and development) which theoretically correspond to the three facets of engaging leadership (i.e., inspiring, connecting and strengthening, respectively). Subsequently, in keeping with the extant body of Job Demands-Resources (JD-R) research, we link job resources to employee work engagement. Our hypotheses were tested on data collected at two time-points – T1 (N = 759) and T2 (N = 273) –from employees working for a hotel chain in the Netherlands. In line with our expectations, engaging leadership showed a significant cross-lagged relationship with autonomy and support from colleagues, but did not predict learning opportunities and work engagement across time. While we formulated specific hypotheses, we also tested reversed causation relationships. We found no direct effect from engaging leadership on employee work engagement, however, the reversed effect was significant; employee perceptions of engaging leadership were shaped by their own engagement experiences. Importantly, engaged employees at T1 reported more job resources at T2. By providing a cross-lagged examination of our model, we showed that engaging leaders as well as employees’ positive affective state of being engaged, are essential to shaping a resourceful work context. A comprehensive view on the triggers and outcomes of work engagement and engaging leadership is needed, as the traditional unidirectional cause-effect rationale fails to explain how these concepts relate to one another and to employee experiences of job resources.  相似文献

针对"互联网+大数据"优化电商配送服务质量问题,明确不同收货方的质量需求稳定性,引导电商根据收货方不同质量敏感性提供相对精准服务,提升配送服务质量。模拟投票结果的形成过程聚类得到收货方对服务质量偏好的记忆性特征:(1)"无记忆"型收货方;(2)"记忆"型收货方;(3)"不确定"型收货方;(4)收货方总体。进一步推导不同规划类型求解空间,设计得到"无记忆"型收货方动态规划精确求解方法,及其他三种类型近似求解粒子群算法。研究过程中,配送资源质量感知度被嵌入到模型约束;"无记忆"型收货方的质量需求规划问题转化为零库存策略最优解问题,进而证明存在精确解;"记忆"型收货方呈现出对质量感知的累积;"不确定"型收货方模型通过赋值即可得总体收货方表达式。结果表明:数据驱动研究框架借助大数据资源,使得电商更易通过收货方的质量偏好设计更加匹配的配送方案;不确定服务需求得到有效满足,投入成本的利用率更高;通过特征分类的方式,尽可能地抽取能够精确最优化的部分,缩小NP范围,提高整体求解的精确度。  相似文献

每当先进的发展制度开始取代落后的发展制度时,整个世界格局就会进入重大的转换阶段。在中国五千年的发展历史中,有两次重大的历史变局完全改变了中国历史发展轨迹并深远地影响了当前中国的发展模式。新时代下的中国作为前两次历史变局影响的客体,曾经既是先进发展制度取代落后发展制度的受益者,也经历过作为落后发展制度主体而被先进发展制度冲击的过程。目前世界正处于第三次先进发展制度与落后发展制度交替的历史变局阶段,作为第三次重大历史变局的主体,在三次千年历史变局叠加的背景下,能否有效地认识、适应并改造利用前两次历史变局的经验与教训,构建系统性、科学性、可行性、领先性的中国特色社会主义制度,将决定中国在本次历史变局中能否顺利成为先进发展制度的主体从而实现民族复兴。  相似文献
